Surgery is the cornerstone for FLC too, but recurrence remains frequent (5-yr RFS ~45%).
For unresectable disease: no validated systemic therapy yet.
Emerging data: DNAJB1-PRKACA-targeted peptide vaccines + checkpoint inhibitors show interesting results

We compared 3 first-line curative treatments for HCC within Milan criteria on cirrhosis:
🔹 Laparoscopic resection (LLR)
🔹 Open resection (OLR)
🔹 Multibipolar radiofrequency ablation (mbpRFA)

🎯 Bottom line: minimally invasive approaches (LLR or mbpRFA) should be prioritized over open surgery for early HCC on cirrhosis.
LLR maximizes oncological control. mbpRFA preserves transplant-free survival with far less morbidity. It's a trade-off, not a hierarchy.
