Happy to share that our multicenter, randomized, sham-controlled phase II trial of low-dose whole-brain radiotherapy in mild-moderate Alzheimer’s disease is now published in Radiotherapy & Oncology! Why we did it: LDRT reduced amyloid and neuroinflammation in AD mouse models. Our two dose arms came from early human reports: a CT-dose case report (0.04 Gy; Cuttler et al., 2016) and a single-arm pilot (0.5 Gy; Kim et al., ROJ 2023). Neither had a control group. What we did: 41 amyloid-PET+ patients were randomized to sham, 0.24 Gy, or 3 Gy (6 fractions over 3 weeks), on top of standard medication. What we found: - 6-month ADAS-cog (primary endpoint) was not met - 12 months (exploratory), −5.4 points with 3 Gy (nominal p = 0.044), though findings were not consistent across other outcomes. No treatment-related adverse events. What’s next: LDRT was safe, but efficacy is not established. Larger, adequately powered trials with longer follow-up are needed, and we look forward to LDRT-MIND from Dr. Evan Thomas, which will add more robust data and explore maintenance. Grateful to our patients, and co-investigators across three centers. 🔗 doi.org/10.1016/j.radonc.202… #Alzheimers #LowDoseRT #ClinicalTrial @EvanThomas84 @jryckman3

Congratulations Dr Kim! Many of us have been waiting anxiously to see your published results! Thanks for your trailblazing work, it served as inspiration for our now accruing trial, LDRT-MIND!

It’s so tough because all oligometastatic is such a humongous spectrum. I just traded a wonderful 40 year-old with oligometastatic esophageal. 4y S/p chemo RT and ivor Lewis, no primary recurrence but w 7 pulmonary Mets that didn’t respond to continued chemo or immuno or histotripsy. CEA climbing, all growing. Largest 4 cm. Single fraction SBRT (26-28Gy) to all 7. 3 mo s/p all w/ >90% volume reduction CEA back to normal first time since cancer diagnosis. There is a true oligo phenotype and the number is not the driver.
The problem with “rendered oligometastatic” is the initial other mets, if they had a 99% response and “gone” by imaging, e.g. 1 cc tumor is approximately 10^9 cells, that’s still 10^7 cells there

I’m so glad to see that we may finally be addressing a question I’ve asked over and over at tumor boards, but never really got an answer to: If the primary tumour is where recurrence often happens, or where the response comes last, why don’t we get rid of it at the first opportunity? Aren’t we asking a right question? Congrats Shankar!
🚨🔔🔔Very happy to announce that we have randomised the first patient into @TOGAANZ and @TROGfightcancer phase III, PRIME-lung trial! First site open @PeterMacCC ➡️ this newly open 420 patient phase III trial investigates whether the addition of high dose radiation to the lung primary in patients with non-small cell lung cancer can be life-prolonging (primary endpoint OS) ➡️ Thank you all investigators and patients, this is just the beginning! #lungcancer #lcsm

It is a basic principle of American democracy that the government can’t selectively target individuals simply because of their political beliefs or what they say. But that’s exactly what the Trump Administration is doing to Congresswoman Ilhan Omar of Minnesota. To understand why this is so appalling, it’s important to know the facts. Congresswoman Omar came to this country with her father when she was 12 years old. Five years later, her father became a naturalized citizen. If Ilhan had been 18 at the time, she would have had to apply for citizenship separately. But because she was 17, she became a naturalized citizen automatically. Congresswoman Omar went on to graduate from college, work as a community organizer, become the first Somali-American legislator elected to public office in the U.S., and win a seat in Congress. There, she has been one of the Trump Administration’s fiercest critics, which has led the president to repeatedly single her out as someone who he believes does not belong in this country. Now the Administration is actively trying to deport Congresswoman Omar, seemingly based solely on the unsubstantiated theory that she was actually 18 when her father was naturalized, and therefore is not a citizen. Federal agents have raided her home, confiscated personal records, and seized her phone — all to try and uncover a nonexistent smoking gun from 20 years ago that will somehow make their case. Think about that. The full weight of the federal government is being brought down on a law-abiding elected official because she has dared to criticize the president. And it’s not the first time this has happened. Under President Trump, the Justice Department has repeatedly gone after those he considers his political enemies — from the Attorney General of New York, to the former head of the FBI, to members of the Federal Reserve. In those cases, the prosecutions either stalled or were dismissed for having no basis in law, and the same is likely to happen in this case. But not before Congresswoman Omar’s life has been turned upside down, her legal bills have piled up, and she’s been forced to spend time fighting unsubstantiated charges instead of serving the people of Minnesota. The fact that any president, or any administration, is willing to engage in such transparent efforts to intimidate someone should outrage all Americans, regardless of how they feel about Congresswoman Omar’s political views. Because if this becomes the new normal, there’s no reason to believe it will stop with her — or that future presidents, in either party, won’t do the same.

If he had gotten SBRT, he would have left 5 minutes after the procedure…. Just saying….
It’s lovely to see @Pontifex leaving the hospital POD#4 after having lung cancer surgery 🫁 on Tuesday. Praying for his fast recovery ( FYI: most patients that undergo robotic lung surgery do leave the hospital within days- surgical techniques have improved)
