The water temp off of Santa Cruz is 68! Unheard of for this time of the year and it’s still getting warmer!
I’m very worried for this winter, it could very well be one for the record books and not in a good way:
Today in San Diego sea level rise news:
➕Midway Dr is closed to thru traffic again due to flooding
➕The river is the highest level I’ve ever seen from I5 to the ocean
➕These levies are not maintained to #FEMA standards so nearby businesses can’t purchase flood insurance

Persistent exertional dyspnea following mild COVID-19: potential association with diaphragm impairment
🚨Mild COVID, ‘normal’ lungs & heart, still breathless 2 years later? Your diaphragm may be the missing piece! #LongC0VID
➡️Interesting small German study of 25 non-hospitalized outpatients with persistent exertional dyspnoea a median of 24 months after mild COVID-19,
➡️Routine cardiopulmonary work-up (lung function, echocardiography, 6-minute walk test, bloods) showed no clear group-level explanation for the breathlessness,
➡️Non-volitional diaphragm strength (twitch transdiaphragmatic pressure (TwPdi) after cervical magnetic phrenic-nerve stimulation) was significantly lower than in matched pre-pandemic controls (median 14.2 vs 21.4 cmH₂O, p=0.02) with 52% of patients falling below the lower limit of normal,
➡️Diaphragm thickness on ultrasound was also reduced at both functional residual capacity (FRC) and total lung capacity (TLC),
➡️TwPdi declined stepwise with worsening dyspnoea severity and correlated strongly and inversely with post-walk mBorg score (r = −0.67),
➡️Fatigue scores were tightly linked to dyspnoea severity,
➡️56% of patients had already had two SARSCoV2 infections and 96% had received ≥2 mRNA vaccine doses, but these factors were not analysed separately.
‼️So, mild COVID-19 can leave the diaphragm measurably weaker and thinner two years later, producing real, progressive breathlessness that ordinary heart and lung tests completely miss.
‼️For the large group of people whose post-COVID breathlessness remains unexplained after standard tests, this identifies a concrete, potentially treatable target (respiratory-muscle weakness). Prior work by the same group has already shown that inspiratory muscle training can improve diaphragm function and reduce dyspnoea in hospitalised longC0VID patients. The authors advocate the same approach may be a plausible alternative for this milder phenotype. Clinically it argues for adding simple respiratory-muscle assessment (or at least considering targeted training) when routine cardiopulmonary evaluation is unrevealing.🤔

After class this morning one of my regulars told me she was going to visit her friend, who was sitting in the ICU with a 36 y/o who has lived with severe cerebral palsy his entire life.
She said it clearly and only once.
"Both of his parents died from COVID 5 years ago."
Her friend was the godmother, so she took him in.
Then almost in the same breath she said, "He recently came down with the FLU, and started vomiting, and because of his cerebral palsy he aspirated and developed pneumonia and sepsis.”
I stood there in my sweaty teaching clothes and watched a window open in her eyes to a deeper connection.
Those eyes suddenly filled heavy with tears. She wiped at them with the heel of her hand, then again, and again. The tears opened a brief, clear window past the word "flu" she kept using and through to the deeper reality of what had already taken his parents.
She kept talking through that reality though, voice catching. "The whole situation is so sad. The poor guy never had a chance once the vomiting started, because he can't swallow with the cerebral palsy."
I thought the next wipe of her hand might come with the real name of what had already taken both of his parents, and has now returned inside that ICU, knocking hard on the door for their son.
The tears kept coming, steady and moved by the story she was telling me. It was the kind of crying that starts once something deeper has already been understood, and the mind is only beginning to catch up.
The word "flu" stayed where she left it, hanging suspended between us.
She never reached back for it, and did not say that word again.
This is the part that still bothers me. It is not the denial exactly. It is how denial and recognition can live within the same person at the same time.
She could say Covid when it belonged to the past, to the two already gone and buried. She could not say it in the present, while the same thing returned for their son, as if the first deaths had only been the beginning.
The word stayed buried with them. What she would not speak came out of her eyes instead.
This is the shape of the world now.
The outline of what remains is still felt on a deep subconscious knowing, and a softer word is offered in its place, so the crying can continue without having to confess what the eyes already know.

Biofire reports SARS-CoV-2 positivity increased to 2.5%, and revised up last week to 2.5%.
This is 25% higher than 1 year earlier.
Verily reports US national, and the West region are lowest since August 8 as of Oct 20.
WWscan reports The West is lowest since August 7 as of Oct 18.
Levels remain significantly higher in the West and South in 2026 vs 2025
2025 levels on Verily:
**October 20, 2025**
**National : 57.2**
**Northeast : 96.0**
**South : 63.0**
**Midwest : 110.1**
**West : 38.1**
The latest Northeast data shows a drop on Verily, however, WWscan has a spike. Coverage is much lower here compared to NWSS in the Northeast, which shows a downtrend

The PNW has had high COVlD levels for approximately 2 months.
If someone does not regularly mask outdoors, a large gathering during a surge would be a wise time to do so. In surges, the odds of *multiple* infectious people in close proximity increases.
Replying to @AriDrennen
Outdoor *crowds* are often super spreader events and the PNW has been in a big surge for a while now. It’s weird that people are so averse to recognize this. Somehow everyone understood the concept of mitigating airborne disease in 2020 and 2021
