Our mission in western 🇸🇻 has begun. Yesterday (Dentist's Day),we conducted a medical/dental care event in El Ranchador,Santa Ana. Initially, check-ups were scheduled for 50 low-income residents of the community. The event was efficiently organized & operated throughout the day.

We began in Enrique's small clinic in El Zonte. Now we're preparing a 2nd clinic kms. away in Santa Ana, bringing the model closer to communities in western 🇸🇻. A treatment room has been identified and a dental chair acquired. The next step is to install & equip it for care 🪥🦷

Monetary and multidimensional poverty in 🇸🇻 is critically concentrated in the western part of the country. We identify certain *cantones* in Santa Ana—and specifically the district of Tacuba (Ahuachapán)—as having some of the highest rates of extreme poverty in El Salvador ⛑️

You sit on the paper-covered table and begin the familiar ritual, repeating your history, listing medications from memory, guessing at dates of past procedures. The clinician nods, types and works with whatever fragments the local system happens to contain. What is missing is not just data. It is the continuous thread of your own body across time. That thread has been quietly claimed by institutions that store it, restrict it and sometimes lose it. The result is care that is forced to start over again and again. Integrity is the first casualty. When records cannot travel freely with the person they describe, every new encounter becomes an exercise in reconstruction rather than progress. Allergies are rediscovered the hard way. Trends that should have been obvious remain invisible. Trust thins. Patients sense they are explaining their lives to systems that do not fully remember them. Outcomes decline not from lack of clinical skill, but from the absence of a complete, trustworthy story. A different approach is already being applied in real hospitals and dental surgeries.
