The study highlights an important shift in surgical oncology: colorectal liver metastases are increasingly assessed not only through anatomy and technical resectability, but also through tumor biology. By integrating clinical variables with genomic and molecular data, the authors showed that molecular-risk stratification and integrated modeling identify clinically meaningful prognostic groups in CRLM.

Hot take: in academic surgery, more papers ≠ more impact. 🔥
@DimitriosMoris @IA_Ziogas @Paulo_MartinsMD argue it's time to move beyond "publish or perish" — and reward depth, mentorship, and innovation over volume.

🏥machine_perfusion (#MP) is increasingly being utilised for #DCD liver transplants.
In this #UNOS study from 2016 to 2025, we showed an accelerated, 7.8-fold increase in the use of DCD livers from donors ≥60 in the #MP_era, with a reduction of the median #waitlist #time from 112 to 62 days, and acceptable early graft survival.

In our recent editorial “Confidence Without Wisdom: The Dunning-Kruger Problem in Modern Surgery” we discussed how confidence, even though indispensable in surgery, if misaligned, it may place patients and professionals at risk.
•Appreciating the perils of an overinflated confidence - especially early on in a surgeon’s career;
•understanding how such behaviours are acquired and reinforced in a system that emphasises decisiveness, productivity and visible individual achievement; and
•learning how it may be recalibrated under proper mentoring and honest self-reflection
are essential for promoting patient safety, professional development, and ethically grounded surgical decision-making.

In our recent commentary in the journal Transplantation, “Transplant Surgery: Sustaining a Workforce at a Critical Juncture,” we address an urgent reality: while abdominal transplantation remains one of medicine’s greatest successes, the specialty itself faces growing workforce instability.
Declining trainee interest, unpredictable 24-hour call structures, misaligned productivity metrics, and promotion pathways that fail to reflect the true scope of transplant work all threaten long-term sustainability.
We outline three structural priorities:
• Redesign clinical operations to improve predictability — including greater use of machine perfusion to enable more daytime scheduling.
• Modernize fellowship training and strengthen early-career support.
• Realign academic promotion metrics to recognize clinical complexity, applied innovation, and systems leadership.
This is not simply about surgeon wellbeing. A stable transplant workforce is fundamental to organ utilization, equity, and public trust.
Sustaining transplant surgery requires structural reform — now.
What is your opinion on that?
