Can a purified botulinum toxin help treat erectile dysfunction? 🤔
This was one of the most fascinating conversations I had at this year's American Urological Association meeting.
For Pearls & Perspectives with Urology Times, Dr. Larry Lipshultz and I interviewed Dr. Nicholas Deebel from Northwestern about his IRB-approved study evaluating a purified botulinum toxin for men with mild to moderate erectile dysfunction.
Here's why I'm so interested...
ED isn't just about blood flow. It's also about who's controlling the blood flow.
We spend a lot of time talking about blocked arteries and venous leak, but the nervous system is what tells the penis when to relax and when to contract. Healthy erections depend on the right balance between the sympathetic ("fight or flight") and parasympathetic ("rest and digest") nervous systems.
The idea behind this study is simple but intriguing: could reducing excessive sympathetic activity help improve erections?
My experience using botulinum toxin for ED is admittedly very limited, but I've seen enough in select patients to make me excited for high-quality data. I also think it's notable that participants in this study will be taking a PDE5 inhibitor, since colleagues who've used this approach have consistently told me the combination appears to matter.
Whether this ultimately changes how we treat ED or not, this is exactly the kind of research I love seeing—asking new questions, challenging old assumptions, and moving men's health forward.

Thanks to @nypost for interviewing me to discuss signs and symptoms of low testosterone!
Social media can point to almost any issue and tell you that it’s because you have low testosterone. Now it very well may be, but in many cases low testosterone is only part of the story. There’s a lot of other health issues that can present similar to low testosterone, but we wont know unless you get evaluated. One of the many reasons to go see a doctor!
