Honored to participate in this important study. It is always challenging to discuss treatment de-escalation, but definitely worth it for our patients. Thanks for phenomenal mentorship @sonbol_bassam @GIcancerDoc and collaboration @sesayo @Faresjamall
This is one of my favorite papers we’ve published , led by our great Mayo resident @WallaceKleinS “Reduced oxaliplatin exposure in gastrointestinal cancers: A systematic review and meta-analysis” We already know from maintenance trials in mCRC that stopping oxaliplatin after 3–4 months is not detrimental for PFS/OS and is associated with less neuropathy. The question in 2026: what is the evidence in mCRC, the adjuvant setting, and other GI cancers? Is there a situation where oxaliplatin is still needed beyond 3–4 months of induction? In other words — is reduced oxaliplatin exposure associated with worse DFS or PFS across GI cancers? We did a meta-analysis across GI cancers and settings: • No statistically significant survival difference overall • Lower neuropathy and grade ≥3 adverse events Take-home: don’t keep oxaliplatin going beyond 3–4 months. These findings support limiting exposure to improve the therapeutic index of oxaliplatin-based chemo across GI settings. sciencedirect.com/science/ar… @OncoAlert @MayoCancerCare #GIonc @GIcancerDoc @WallaceKleinS

Standard vs. reduced dose of everolimus for patients with advanced NETs...? A retrospective study of 170 patients from Mayo Clinic and University Hospitals in Cleveland. @sonbol_bassam @Faresjamall @PatrickMcGarrah and others not found on X. Key findings: -- Patients in the reduced-dose cohort were older, with worse PS and more heavily pretreated -- Median OS was shorter in the reduced dose cohort, 43.7 vs. 81.5 months (p=0.02) -- After multivariate adjustment, the difference was no longer statistically significant -- Similarly, the observed difference in PFS (7 vs. 12.4 months), was no longer significant after adjusting -- Almost half of the full-dose starters needed dose reduction Lower doses may be reasonable in patients expected to have poor tolerance but cannot be routinely recommended. A prospective trial (NCT06472388) is ongoing, led by @RachelRiechelm2 onlinelibrary.wiley.com/doi/…

Proud to present 5 studies at the #ACG2026 meeting and win the presidential award. The studies included ASUC prediction model, coccidioidomycosis infection in the setting of IBD treatment, novel treatment strategies in IBD, and zolbetuximab-associated hypoalbuminemia in GEA. Grateful for all the mentors and collaborators @sonbol_bassam #Talhamalik #IBD #GEA #GI #nashville

Out now in @JCOOP_ASCO: our SR/MA of neutropenic diet (ND) vs liberalized diet (LD) in patients with cancer. 15 studies, 4,320 patients. ND vs LD: • Major infections: RR 1.02 (0.88–1.18) • Bacteremia/fungemia: RR 0.97 (0.78–1.22) • Pneumonia: RR 0.98 (0.59–1.63) • Diarrhea: RR 1.12 (0.96–1.31) • Mortality: RR 0.94 (0.77–1.15) RCT-only analyses were consistent. In heme/HSCT studies overall (9 studies; n=1,769), no difference across outcomes. One signal to not over-read or ignore: in RCTs limited to heme/HSCT (6 studies; n=855), ND was associated with lower bacteremia/fungemia (RR 0.67, 0.47–0.95), with no difference in the other endpoints. Bottom line: these data do not support routine ND for short-term neutropenia or lower-risk populations. Selected high-risk patients — particularly HSCT or prolonged neutropenia from hematologic malignancy — may still warrant a more cautious approach. Grateful to present our work at #DDW2026 #DDW GLP-1 receptor agonist use was associated with a decreased risk of IBD-related surgery compared to non–GLP-1 use. We also observed a significant reduction in HbA1c in the GLP-1 group. IBD related hospitalizations trended lower with GLP-1 use but did not reach statistical significance. Thankful for the mentorship and support from Drs. #talhamalik and #nathandelafield . Looking forward to building on this work. @MayoClinicGIHep @MayoAZ_IMRES #mayoclinic #mayo #gastroenterology #IBD #research

Always an honor working and learning from you @sonbol_bassam Studying how liquid biopsy correlates with tissue biopsy was really interesting! Many more to come! #biopsy #liquidbiopsy #cancer #GIcancer
🧵 How well does ctDNA testing perform compared to NGS/IHC on tissue from metastatic site or primary site in in gastroesophageal cancer? This is one of my favorite projects led by our superstar research fellow, soon to be @MayoCancerCare IM resident @Faresjamall In our @JCOPO_ASCO (n=345), ctDNA showed high concordance with metastatic site (example ERBB2 97%) and 78.8% for ERBB2 for primary site. Here’s what this means for real-world care 👇 #ctDNA #LiquidBiopsy #PrecisionOncology #GastricCancer @OncoAlert @OncBrothers 1/8

Can't agree more with the discussion below! Replacing these pancreatic enzymes is important not just for symptomatic relief, but also to decrease the chance of oxalate nephropathy! This just came out in @BMJCaseReports where we describe a pt with #NET who d/t financial constraints was unable to consistently take pancreatic enzymes and had worsening kidney function; we confirmed the diagnosis of oxalate nephropathy as below. Led by @Faresjamall casereports.bmj.com/content/… As stated below, diarrhea in #NET is multifactorial but I typically think of three main things that need to be ruled out: 1- Secretory: get plasma 5-HIAA (has its limitation); tx: treat the disease +/- short acting SSA +/- telotristat 2-Bile acid diarrhea (BAD): get 7AC4; if abnormal, then colestipol (more pt friendly than cholestyramine). Note here: BAD can happen even in the absence of prior bowel surgery in #NET pubmed.ncbi.nlm.nih.gov/3436… 3-Pancreatic insufficiency: stool pancreatic elastase. If abnormal, then tx with pancreatic enzymes as prescription if possible (BIG problem here with $$; it is ridiculous that such therapy can be hundreds of dollars even with insurance! as @OncoThor mentioned, OTC options are available but unfortunately I only had rare success w that)
Exocrine pancreatic insufficiency remains one of the most under-recognized and therefore under-treated conditions in oncology. At my current institution, I work closely with an expert dietician, and her thoughtful approach to symptom assessment and management has significantly improved recognition of EPI in my clinic. It is so important to understand that most patients will not have classical EPI symptoms, in fact there are no classical symptoms at least in my experience. We now initiate pancreatic enzyme replacement therapy (PERT) in at least 1–2 patients per week, which is a substantial increase from my prior practice. This is truly multidisciplinary care in action!!
