🎖️Can we predict STER difficulty before entering the tunnel? 📣Proud to share our latest publication; 👉 thieme-connect.com/products/… “Submucosal Tunneling Endoscopic Resection for Upper Gastrointestinal Subepithelial Lesions: Development of a Digital Nomogram” 📕In one of the largest single-center STER experiences reported to date (150 consecutive patients), we evaluated procedural outcomes and developed a practical risk prediction model for pre-procedural planning. 🔑Key findings: 🔹 100% technical success 🔹 85.3% en-bloc specimen retrieval 🔹 Longitudinal tumor length was the strongest predictor of procedural difficulty and piecemeal retrieval 🔹 Deep muscularis propria origin independently predicted complications 🖍️Importantly, we identified clinically relevant size thresholds: 📏 >34 mm → increased complication risk 📏 >38 mm → increased likelihood of a difficult procedure 📏 >43 mm → high probability of piecemeal retrieval 📏 >50 mm → nearly 90% likelihood of requiring piecemeal retrieval ✨To facilitate individualized risk assessment, we also developed an interactive digital nomogram and STER risk calculator. 👉 nomosmed.streamlit.app ✅As third-space endoscopy continues to evolve, patient selection and procedural planning are becoming as important as technical execution. 👏Congratulations to all co-authors and our entire endoscopy team for their contributions to this work.@ESGE_news @ASGEendoscopy @my_ueg @kocuniversity

⭐️From Pascal to Poiseuille: A Physiology-Driven Approach to Epiphrenic Diverticulum 🎖️The scientific outcome of our epiphrenic diverticulum case — presented at #DDW 2025, San Diego and honored with the Best of the Best award — has now been published. 💥This work represents more than a technical description; it reflects an effort to rethink the pathophysiological foundations behind therapeutic decision-making. In the light of the principles of Pascal, Laplace, Bernoulli, and Poiseuille, we explored how pressure balance, flow dynamics, and wall tension can guide endoscopic strategy. ☄️Different from our previous experiences, in this case we shared an approach performed without septotomy, aiming to restore intraluminal esophageal pressure equilibrium while enabling esophageal reconstruction. 🔭The physiological rationale, scientific framework, and clinical implications of this concept are discussed in detail in the discussion section of the manuscript. 💡In addition, we believe that integrating suturing strategies into advanced endoscopic resections may represent more than a technical step — potentially accelerating a paradigm shift in therapeutic strategy. 👏Grateful to all colleagues, mentors, and team members who contributed through discussion, collaboration, and shared thinking — because progress in advanced endoscopy is rarely an individual effort, but a collective journey. 🙇Sometimes in advanced endoscopy, the real limits are not defined by the devices we use, but by how we interpret physiology. 📚Guided by science, we continue to pursue more rational and safer minimally invasive solutions for our patients. 📝& 🎥Article and Movie : sciencedirect.com/science/ar… @DDWMeeting @ASGEendoscopy @GIE_Journal @ESGE_news @my_ueg #DDW2026

⭐️Where Is the Limit? 🎯Stricture Prevention After a 33-cm Circumferential ESD 🎖️Our study, which represents the scientific counterpart of the video selected for the DDW 2025 – ASGE World Cup Sessions, has now been published. In this article, we detail: ✅ our approach to large circumferential ESD, ✅ strategies for stricture prevention after extensive resections, and ✅ the technical rationale of endoscopic suturing in this setting. 📌A case-controlled study with long-term follow-up and advanced analyses will be shared soon. 📌A rectal LST-GM was referred for endoscopic treatment. Yet, advances in imaging technology now allow more precise staging and guide us toward the right therapeutic approach. 🎯Although biopsies appeared benign, targeted diagnostic sampling during endoscopic evaluation revealed the need for surgery.
