NEWLY PUBLISHED: ALLOGENEIC STEM CELL SAFETY PAPER 🔬 305 patients. Zero treatment-related serious adverse events. 📄 doi.org/10.13052/ijts2246-87… New peer-reviewed safety data on Cellcolabs allogeneic (donated) bone marrow-derived stem cells (StromaForte), from two ongoing clinical trials in The Bahamas. THE SAFETY DATA ✅ 305 treated participants ✅ 329 patient-years of follow-up ✅ No treatment-related serious adverse events ✅ No discontinuations due to side effects ✅ No DVT, pulmonary embolism, sepsis or joint infection ✅ No new safety signal Most side effects were minor, mainly short-lived blood pressure rises around dosing. These are interim results from open-label studies, and the authors rightly describe them as provisional while follow-up continues. THE TRIALS Musculoskeletal: clinicaltrials.gov/study/NCT… Cardiovascular: clinicaltrials.gov/study/NCT… THE INTRAOSSEOUS TECHNIQUE Dr Steven Sampson and Dr Rikin Patel trained in intraosseous (IO) delivery, where cells are drilled directly into the bone, with its pioneer, Dr Mikel Sánchez, through the TOBI network. They have been high-volume users of the technique ever since, which is why they took part in these trials. pubmed.ncbi.nlm.nih.gov/3070… CHRIS'S STORY 🎾 Chris, a keen tennis player, had been through several knee operations and was facing a high tibial osteotomy. Instead he joined the Bahamas trial, where Dr Sampson delivered the stem cells intraosseously. A year and a half of pain disappeared within five weeks. Ten weeks later he was back on court. Two years on, he's still playing and has become a dad. 💙 When a photo of Chris's prepared stem cells was first shared online, it caused quite a storm. This published safety data now gives the full picture. THANK YOU 🙏 To the authors: Dr Darius Bain, Dr Steven Sampson, Dr Danielle Aufiero, Tonia Lightbourn Robinson, Gillian Lum, Dr Cara Dorsett-Pena, Dr Rowan Paul, Dr James Ghattas, Dr Nathan Thakur and Dr Rikin Patel (Principal Investigator). To Cellcolabs Clinical, whose GMP-certified cells are built on more than 20 years of research from the Karolinska Institutet. The trials are self-funded, which is why many people who wanted to take part could not. And to STORZ MEDICAL, whose electromagnetic focused shockwave therapy and EMTT were used before and after Chris's stem cell treatment. I have used STORZ MEDICAL focused shockwave since 2018. Shared with Chris's full consent. Every case is different and results cannot be guaranteed. #StemCells #RegenerativeMedicine #ShockwaveTherapy

ONE SKI FALL. ONE BUSY CEO. NO TIME FOR ENDLESS APPOINTMENTS. 20 February 2024: a CEO came to me after a skiing fall with a significant shoulder injury. He runs a business and had no time for waiting lists, separate scans, separate specialists or endless appointments. He needed to get back, and it needed to be fast and effective. THINKING OUT OF THE BOX That's how he runs his company: finding solutions, not accepting delays. So that's how we approached his shoulder. Imaging together from the start. Then focused shockwave and EMTT, twice a week. No needles, no anaesthetic, no downtime, and no time lost from work. Rehab kept lean and efficient, fitted around his life rather than the other way round. THE RESULT 23 April 2024, just nine weeks later, he was skiing the Argentière glacier in Chamonix, then the Italian side of the Vallée Blanche on Mont Blanc. WHY IT WORKS This is exactly why I've ended up looking after so many in-season athletes and busy professionals. Many hold down demanding full-time jobs alongside training and competing. They simply don't have time for endless appointments. So we changed the order:
• Gather the history, examination and all the scans together at the start
• Begin with non-invasive focused shockwave and EMTT
• Keep them moving: bikes and the pool rather than long, rigorous rehab programmes
• If anything doesn't respond, the other doors stay open: orthobiologics, and surgery for the few who genuinely need it THE EVIDENCE KEEPS GROWING My bibliography now holds 705 published studies on shockwave, EMTT and TPS across 10 clinical areas. Over 200 were published in each five-year block since 2015, and a leading European journal has just given shockwave a whole special issue on regenerative medicine. Key references are in the replies. Worth knowing about.

The 3D learning we saw demonstrated by the ISMST faculty in Madrid has now launched as the ISMST Online Shockwave Certification Course, showing how shockwaves behave in the body and how each treatment is applied. It runs over 14 modules with a protocol library based on the ISMST consensus, and leads to the official ISMST certificate: wavevision.at/

Looking forward to going to the closing concert of the Edinburgh International Festival tonight with the Berliner Philharmoniker at the Usher Hall — their first return to the Festival in 20 years. Kirill Petrenko conducts, with Augustin Hadelich the soloist in Beethoven’s only Violin Concerto. Hard to believe now, but its 1806 premiere fell flat — the violinist reportedly sight-read it on the night — and the piece languished for nearly forty years until a teenage Joseph Joachim revived it and secured its place among the greats. After the interval, Scriabin’s Third Symphony, “The Divine Poem” (1905): the moment this Russian mystic — a man who saw music in colour and believed it could transform the very soul — broke free of tradition and began reaching for his own ecstatic, cosmic language. From Beethoven’s serenity to Scriabin’s rapture. What a way to end the festival. 🎻✨

FROM PROMISE 2025 IN EDINBURGH TO COLORADO SPRINGS Two remarkable days, and a real privilege to be among such an impressive community. Having been at PROMISE 2025 in Edinburgh, seeing it grow into this — 229 delegates from 18 countries at the U.S. Olympic & Paralympic Training Center in Colorado Springs — has been quite something. It has essentially doubled in size in just one year. Mental health in elite sport is at last being given the serious, honest, evidence-led attention it deserves, thanks to a community that keeps turning up for it. Congratulations to all involved. Already looking ahead to 2027. #PROMISE26

Just discharged from NHS orthopaedic follow-up. This is what focused shockwave and EMTT can do for bone healing when they are given the chance. A 67-year-old patient of mine fell on ice in January and sustained a right proximal humerus fracture with varus impaction. At her 12-week NHS review, plain X-rays showed limited callus progression. Her shoulder surgeon was completely convinced she was heading for a non-union and formally proposed a reverse total shoulder arthroplasty, citing return to swimming as the justification. She was frightened. She declined the surgery in writing and came to me for adjunctive biological treatment at 15 weeks post-injury. Protocol: 13 sessions of focused ESWT (Storz DUOLITH SD1) targeted at the fracture site, combined with EMTT at 12,000 pulses per session, graduated energy, no anaesthesia. DEXA arranged via her GP revealed severe osteoporosis (lumbar T-score −4.1) — a finding that had been missed entirely on the NHS pathway. Her shoulder healed anyway. 21 July 2026: NHS CT scan confirms total bony healing. Pain-free. Full active range. Swimming 20 lengths. Back working on her boat. Native joint preserved. Discharged. This is not luck. It is the biology reaffirmed weeks earlier at the 28th World Congress ISMST, Madrid, in Rainer Mittermayr’s keynote “Shockwaves that heal: 25 years of success in fracture care,” chaired by Wolfgang Schaden. Four decades of Austrian and international peer-reviewed research: 75–80% union rates in long-bone non-union, via mechanotransductive upregulation of BMP-2/7, VEGF, and osteoblast pathways. Focused shockwave does not heal bone. It signals bone to resume healing. (Anonymised. Full patient consent.) #ISMST #ShockwaveTherapy #BoneHealing #EMTT #Osteoporosis
