My husband and I took our four-year-old daughter, Maya, to a children's hospital in Columbus for a developmental evaluation that took us five months to book. The appointment was at 9:00 a.m. We checked in at 8:40 and sat down with a bag of crackers, two picture books, and a very patient child. I told Maya, "It's a big hospital, so we might wait about an hour." At 10:00, nobody called us. At 11:00, a family that arrived after us was called back and left an hour later, smiling. I walked up to the front desk. "Hi, we've been here since 8:40. Can you check on our appointment?" The receptionist glanced at her screen. "The doctor is running behind. Just a little longer." At noon, Maya was lying across three chairs with her shoes off. I went back. "It's been three hours. Is she still on the schedule?" "She's on the schedule," the receptionist said, and went back to typing. At 12:15, a nurse finally came out with a clipboard and gave us a bright, tired smile. "I'm so sorry. The doctor has gone to lunch, so Maya will be seen this afternoon." I took a breath. "We have been here since 8:40, and I have asked twice. I'm not asking for special treatment. I'm asking for the appointment we already have." "I understand. If you could come back at 1:00, you'd be first." So we went to a sandwich shop across the street. Maya ate half a grilled cheese and asked, "Is the doctor mad at me?" I almost cried into my coffee. At 12:55, we were back in our seats. At 1:00, we were first in line. At 4:00, we were still first in line. At 4:10, the same nurse came out, and this time she didn't smile. "I'm so sorry. The doctor had an emergency and had to leave. Could you reschedule?" I looked at her for a long moment. "When is the next opening?" She typed. "March." It was October. I looked down at Maya, who was asleep against my arm with a cracker still in her hand. My husband, who had taken the day off work to be there, set his jaw but didn't say a word. I stood up very slowly. "Thank you for being honest with me," I said. "Could you please tell me the name of the patient advocate on duty today?" She blinked. "The... patient advocate?" "Every hospital has one. I'd like to speak with them before I leave." She hesitated, then picked up the phone. Ten minutes later, a woman in a navy blazer walked into the waiting room. Her name tag said "Patient Relations." She listened to the entire story without interrupting, then pulled up the schedule on a tablet. Her face changed. "Mrs. Parker, our system shows that Maya's appointment was marked as a no-show at 9:15 this morning." My husband and I stared at her. "We checked in at 8:40." "I can see that now. The check-in was never entered. That's why nobody called you, and that's why the clinic never knew you were here." She turned the tablet toward us. In the notes field, someone had typed, "Family did not arrive." For a second, I couldn't speak. Seven hours. Two rounds of asking. One sleeping child. And the whole time, we had been invisible. "I'm going to fix this," the woman said quietly. "Not in March. Please give me a moment." She stepped away and made two calls. When she came back, she was holding a different clipboard. "Dr. Alvarez, our division chief, is still in the building. She has agreed to see Maya right now, and she will personally review your daughter's full evaluation. We will also waive all fees for today and coordinate the follow-up visits for you." My husband finally spoke. "Thank you." It was the first thing he'd said in two hours. Dr. Alvarez was kind, unhurried, and thorough. She sat on the floor with Maya and built a tower of blocks for forty minutes. When she was done, she turned to us and said, "Your daughter is delightful. Here is what I see, and here is what we'll do next."

OpenAI models helped researchers—operating as part of our Patch the Planet initiative—discover critical security flaws in PHP, an open-source programming language used to run an estimated 70% of websites. Over the past two months, we led a coalition of maintainers, researchers, and security providers to prepare fixes and protections ahead of disclosure. This duration was necessary because the fix was quite intricate and required a major change to the PHP engine. Every site using PHP can now safely upgrade their version to stay secure, in addition to applying mitigations from various security vendors. Due to the severity of these flaws and PHP’s widespread use, we consider this bug to be one of the most impactful vulnerabilities in recent memory. It also marks a new phase, where AI can find security bugs with internet-scale impact. This is an example of the Defender’s Window in action: to our knowledge, frontier AI was able to find this vulnerability ahead of attackers, giving defenders time to prepare. openai.com/index/coordinated…

Well, boys, here it is, the October surprise. Amy Acton is cooked after this one. I wonder if the betting odds on Kalshi have adjusted to this yet because I’m about to throw some money down. I think we can safely say Vivek will be our next governor in the state of Ohio.
