CLINICAL CASE 🩺
A 24-year-old man presents to the emergency department with severe abdominal pain that began around his umbilicus 18 hours ago. The pain later shifted to the right lower quadrant. He has vomited twice and has lost his appetite.
On examination, he looks acutely ill. His temperature is 38.6°C, pulse 124 bpm, BP 88/56 mmHg and respiratory rate 26 breaths/min.
His abdomen is tender in the right iliac fossa, with involuntary guarding and rebound tenderness. His extremities are cold, and capillary refill takes 4 seconds.
He is haemodynamically unstable, with signs of shock and peritoneal irritation.
Your tasks:
1. What is your leading diagnosis, and what complication are you concerned about?
2. What are your immediate management priorities?
3. Would you request a CT scan before involving the surgical team? Explain your decision.
You are the first doctor to assess him. What do you do next?
