In this Scientific Perspective, Scheperjans and colleagues provide an insightful overview of how the gut has evolved from a largely overlooked contributor to non-motor symptoms into a central focus of biological models of Parkinson’s disease. A must-read for anyone interested in the gut–brain axis and the future of PD research! loom.ly/btphAuw
Could targeting TGF-β signaling offer a potential therapeutic strategy for progressive supranuclear palsy (PSP)?
In this interesting study, Talarico and colleagues demonstrate that TGF-β inhibition attenuated tau hyperphosphorylation, dopaminergic neuronal loss, and astrocytic reactivity in PSP patient-derived organoids, offering a promising avenue for further investigation of disease-modifying therapies. loom.ly/5lHcyVk
In this article, the authors highlight a potential association between vitamin B6 deficiency and seizures in people with Parkinson’s disease. Among 144 patients studied, 3 had suspected epileptic events with B6 deficiency; 2 died following refractory status epilepticus.
CLINICAL REMINDER: Maintain a high index of suspicion for B6 deficiency when patients with PD develop new seizures or paroxysmal events. Early recognition and timely treatment may be critical.
[The FDA is now requiring updated warnings for carbidopa/levodopa products regarding vitamin B6 deficiency and associated seizures.] loom.ly/L2oYhhE
Evidence-based review analyses and clinical practice guidelines are complementary, not competing, tools. The @movedisorder EBM and EAN/MDS-ES Guideline committees explain how using such tools together leads to more informed patient-centered decisions. movementdisorders.onlinelibr…
A low rate of women senior authorship among neuro subspecialty journal articles has not improved since 2014, despite notable increases in overall women authorship. @movedisorder #WomenInMD

An intriguing 15-year follow-up of patients with isolated generalized dystonia treated with bilateral GPi-DBS.
The therapeutic trajectories diverged considerably over time, with only 41% maintaining meaningful benefit. Strikingly, 94% developed parkinsonian signs, predominantly axial, and 67% reported freezing of gait.
Whether this reflects disease evolution, chronic stimulation, or an interaction between the two remains an important unanswered question. loom.ly/tpE0xPY
Sleep architecture and circadian rhythms are profoundly altered in Parkinson’s disease, but can we actually decode them from the basal ganglia?
This proof-of-concept study used chronic ambulatory STN recordings from implanted neurostimulators alongside wearable sleep data. Beta, gamma, and low-frequency activity showed distinct relationships with sleep stages and circadian states. An exciting step toward neurophysiology-guided, sleep-aware adaptive DBS. loom.ly/XbsG2To
An excellent viewpoint on the long-duration response (LDR) to levodopa—an intriguing yet underrecognized and under-investigated phenomenon in Parkinson’s disease.
The authors discuss how LDR can be measured, the evolution from withdrawal-based to model-derived approaches, and its potential implications for understanding disease progression, neuroplasticity, disease modification, and even DBS outcomes. loom.ly/xpoI2bs
Pragmatic diagnosis and nonpharmacologic management of apathy in Lewy body disorders relies on differentiating apathy from depression, assessing cognition, and implementing structured activity, per consensus within a Non-Motor Symptoms Study Group project workgroup. movementdisorders.onlinelibr…
An important study by Leocadi and colleagues provides the first longitudinal evidence of early cortico-striatal and cerebellar functional network alterations in adult Huntington’s disease gene-expanded (HDGE) individuals—changes detectable decades before clinical motor onset. loom.ly/PSn6o24
Apathy is one of the most prevalent yet underrecognized non-motor symptoms of Lewy body disorders. It affects motivation, reward processing, emotion, executive function, and self-initiation—and is associated with faster cognitive decline, greater functional dependence, poorer quality of life, and increased caregiver burden.
A timely position paper by Kulisevsky et al. provides an excellent framework for understanding, assessing, and managing apathy in PD and DLB—and highlights how much remains to be done therapeutically. loom.ly/QDeWHwI
Are there really meaningful differences between α-Syn-SAA–negative and –positive Parkinson’s disease?
Brooker and colleagues report that, apart from greater hyposmia in the SAA+ group and more subcortical atrophy in the SAA− group, the phenotypes appear remarkably similar.
Here is the original article, along with a thought-provoking correspondence by @AlbertoEspay and colleagues. @ajlees loom.ly/qBHAVTM
Does cannabis-based oil improve pain in Parkinson’s disease?
A phase II, randomized, placebo-controlled trial by Kubota et al. found no significant improvement in pain measured by the PD-Pain Classification System with a CBD/THC formulation (CBD 96 mg/mL; THC 2.1 mg/mL). loom.ly/VYh5T5M
In this important study, repeat MRI-guided focused ultrasound thalamotomy restored meaningful tremor control in patients with ET and tremor-predominant PD. Although repeat treatment was associated with more side effects, 85% of patients would choose retreatment again. Importantly, tremor recurrence can occur even after repeat treatment, with progression rates similar to those after the initial procedure. loom.ly/ngrFPw0

New data from a multicenter longitudinal PAF cohort: MDS prodromal MSA criteria identified nearly all future MSA cases (92–100% sensitivity), but specificity was modest. Patients who phenoconverted to MSA were younger and had more severe urinary dysfunction and greater orthostatic heart rate responses. These findings may help refine early MSA prediction and clinical trial enrollment. loom.ly/sjAsOec
In this interesting study, Puisieux and colleagues report significantly higher plasma homocysteine levels in both symptomatic and premanifest Huntington's disease compared with controls. Hyperhomocysteinemia was present in nearly half of patients (48%) and was associated with more rapid motor progression (P < 0.001), highlighting a potential role for one-carbon metabolism in HD. loom.ly/rEgKvAE
A must-read article by Corcos and colleagues for anyone caring for people with Parkinson's disease. The authors make a compelling case that exercise and lifestyle modification aren't just complementary therapies, they're essential parts of Parkinson's disease management. loom.ly/VInVkRM
