The buzz about retatrutide and the rapid uptake in GLP-1 use for weight loss could drown out the message that obesity is a complex, multifactorial disease requiring individualized, long-term management and instead shift focus to a public discourse on which drug helps people lose the most weight the fastest. Patients may be drawn to rapid weight loss, but treatment goals should be gradual and tailored to each patient’s health needs, personal circumstances, and preferences, with an emphasis on sustainable, long-term health improvements, according to the American Diabetes Association. mdsc.pe/4jBadCC

In the week ending October 10, 5% of flu tests run by clinical labs came back positive, up from 3.9% the week before, according to the CDC’s first weekly flu report of the season. The West Coast is seeing the most activity, and nearly all of it is H1N1, which made up 96% of the influenza A viruses the public health labs subtyped. The CDC said activity is higher than usual for this time of the year, though still within normal range of seasonal variation. The basic clinical approach hasn’t changed in view of an early flu season, two experts told Medscape Medical News. But the early start may have caught many patients before their flu shot, putting more weight on testing and antiviral decisions that are rarely clear-cut. mdsc.pe/488xCUR

If you find it hard to keep patients exercising, you’re not alone: even in carefully designed studies, attrition ranges from 25% to 50%, and those who finish adhere only about 66% of the time. Motivational messages usually focus on the benefits of starting, such as a stronger heart, better balance, lower blood pressure, and improved brain health. But what happens when you stop? That may or may not motivate someone, but understanding the cellular effects of quitting could help patients recovering from injury or surgery, undergoing cancer treatment, or on bed rest, as well as people taking a break after a goal like a marathon. Updates to an FDA-approved colorectal cancer blood test increased the detection of advanced precancerous lesions, a new study showed. “The updated test has improved performance, better detection of advanced precancerous lesions, resulting in larger projected reductions in CRC incidence and mortality at the population level,” said first author Aasma Shaukat, MD, MPH, of the NYU Grossman School of Medicine in New York City. Although colonoscopy and stool-based testing continue to be recommended as first-line screening in American Cancer Society guidelines, “for patients who are unwilling or just will not proceed with screening, we can consider offering them a blood test,” said Shaukat, who presented the findings at the American College of Gastroenterology 2026 Annual Scientific Meeting. Up to 80% of systemic sclerosis patients develop pulmonary or cardiac involvement, yet care often remains fragmented across isolated specialties. Managing a complex, multi-system autoimmune condition requires more than independent consultations—it demands coordinated care pathways between rheumatology, pulmonology, cardiology, and dermatology. When care is siloed, organ involvement progresses undetected. Establishing strong referral pathways to designated Scleroderma Research & Treatment Centers is critical for long-term patient outcomes. That’s why Medscape supports the National Scleroderma Foundation Rocky Mountain Chapter and their efforts to champion integrated, multi-disciplinary care models. 🎥 Watch the full "Rare, but Resilient" discussion between Dr. Tracy Frech, She Shutes (Patient Advocate), and Medscape Education. A quality improvement project nearly quintupled the rate of children receiving evidence-based, shorter courses of antibiotics for pneumonia and sinus infections, cutting unnecessary drug exposure without increasing treatment failures, according to the researchers. Research increasingly shows shorter courses of antibiotics — 3-5 days for community-acquired pneumonia and 5-7 days for acute bacterial rhinosinusitis — are safe in children. But clinical adoption has lagged. mdsc.pe/4elyvwM

A meta-analysis found that structured obesity management training programs for healthcare professionals (HCPs), defined by clear learning objectives and curricula, were significantly associated with improved HCP competencies — especially confidence. Only the longer programs (≥ 4 hours) were associated with a significant improvement. mdsc.pe/3U5tRMH

Every doctor can get bogged down with an endless daily to-do list consisting of everything from pre-authorizations and prescription refill requests to handling in-box messages from the patient portal, and endless electronic health record tasks. These critical (but often invisible) tasks done “behind the scenes” usually take place before patients arrive in the early hours of the morning or at home late into the night — and are drivers of both burnout and exhaustion, with much of this non-patient work spilling into weekend and weeknight hours. mdsc.pe/47CtfkL
