H.R. 1 (a.k.a. the “One Big Beautiful Bill Act”) is taking effect now. Changes to Medicaid and other federal health coverage programs could impact how patients get and maintain coverage and access care. Here’s what’s changing now and what’s ahead:

What does Medicare physician payment have to do with whether you can get an appointment with your doctor? More than most patients realize. The cost of running a medical practice keeps rising, and when Medicare payments fail to keep pace, independent practices can face increasingly difficult decisions about whether they can continue serving Medicare patients or remain open at all. That is why Patients Rising is supporting the Patients First Act, H.R. 9693, which would change how Medicare physician payments are calculated and provide more predictable annual updates tied to the cost of delivering care. Because for patients, this is not just a payment formula. It can mean whether your doctor’s door is still open when you need it. Ask your representative to support H.R. 9693, the Patients First Act.

Getting an appointment shouldn’t depend on whether your doctor can afford to keep the doors open. Yet across the country, patients are waiting longer, traveling farther and watching independent practices disappear from their communities. Medicare physician payment, adjusted for practice-cost inflation, has fallen 33% over the past 25 years. For patients, that erosion can eventually show up as fewer doctors, fewer appointments and fewer options close to home. The Patients First Act (H.R. 9693) takes aim at the payment policies contributing to that problem, including annual reimbursement updates and Medicare’s budget-neutrality rules. Patients Rising is supporting the legislation and bringing the patient voice into a debate that can determine whether care remains accessible in the communities that need it. An important issue could have real repercussions in healthcare: A $103,000 visa fee could have a very real impact on whether some communities can access quality care from a doctor. That’s why the AMA is urging the Administration to exempt physicians from the proposed H-1B fee. 1/3

94% of pharmacy benefit manager (PBM) markets were highly concentrated in 2024. And concentration is growing – which can have far-reaching consequences for patients and what they may pay out of pocket.

AMA President, Willie Underwood III, MD, MSc, MPH, was in Washington D.C. yesterday to support H.R. 9693, the Patients First Act. This legislation will preserve patients’ access to care and physician practice sustainability by connecting Medicare payment to the actual costs of running a practice. Also this week, the AMA and state medical societies representing physicians from all 50 states and the District of Columbia sent a letter in support of this legislation. The debate over Medicare reimbursement can sound like an argument over payment formulas until you consider what those numbers mean for the patients trying to get an appointment. Patients Rising Chief Mission Officer Terry Wilcox takes that argument to the pages of The Wall Street Journal, where her letter examines the growing divide between what Medicare pays physicians and what it costs to deliver care. Medicare physician payments have fallen 33% over the past 25 years when adjusted for practice-cost inflation. Terry argues that years of underpayment are reshaping where physicians can afford to practice and ultimately how readily Medicare beneficiaries can access care. Her letter also raises a larger question for Congress as calls for Medicare for All continue: How can we promise to expand a system that is already struggling to adequately pay the physicians caring for the people enrolled in it? Patients deserve access to care, and physicians need a Medicare payment system that reflects real practice costs. The AMA and medical societies in all 50 states + DC are urging support for the bipartisan Patients First Act (H.R. 9693). #FixMedicareNow spr.ly/6010BGuNqo

When Medicare payments fall further behind inflation, patients pay the price with longer waits, fewer physicians and worse access to care. Congress must act before the system falls even further behind. spr.ly/6019BGBVaS

For too long, Medicare Advantage plans have misused prior authorization to delay or deny medically necessary care. Legislation adopted by the House Energy and Commerce Committee today would bring greater transparency and accountability to the #prioraurth process and reduce burdens that stand in the way of patient care. AMA is committed to working with lawmakers of both parties until patients are no longer denied medically necessary care. @HouseCommerce @EnergyCommerce spr.ly/6018BElARK

As a physician who has served patients for 35 years, I am sickened by the unconscionable practices of health insurance companies that continually place profits above patient care. Prior authorization, in particular, is being abused to prop up profits and deny care to those who need it.

Despite last summer’s insurer pledge to reduce prior authorization (PA) burdens, only 1 in 3 physicians believe it will have a meaningful impact. ➡️ 95% say PA delays care ➡️ 94% say PA increases burnout ➡️ 26% report PA has led to a serious adverse event for a patient Pledges aren’t enough. Patients deserve better. #FixPriorAuth Read the full results: spr.ly/6012BBvcFM

Low competition and high vertical integration in the PBM industry places vast power and control over medication, access and affordability in the hands of a few insurer-owned PBMs. An AMA analysis provides insight to help concerned lawmakers understand the anticompetitive conditions in the PBM market that can cause harm to consumers. spr.ly/6010C768O

No surprise @repBuddyCarter is frustrated with costly delays from #priorauthorization. Hear more Georgians’ stories: fixpriorauth.org/stories?sta… Congress must act—pass H.R.3514 & #FixPriorAuth now! #PassTheSeniorsAct
Doctors—NOT health insurance companies—know what's best for their patients. When a doctor or pharmacist says a patient needs treatment, health insurance companies should listen. Denying lifesaving care after patients pay so much is shameful and they know it.
