Great piece explaining current profusion of diagnostic fads & obstacles to corrective "de-diagnosis".
3 causes:
1)Demand led self-diagnosis rather than careful clinical evaluation 2)Identity-conferring rather than descriptive labeling 3)Self-reinforcing rather correctable

Some therapists work to develop their clinical skill.
Some work to build their online brands.
It’s probably good to recognize the difference.
So under-researched, post-partum conditions are relegated to psychiatry, the last resort for unusual behaviors, instead of being addressed by gynecological endocrinology. Why are women treated this way?
This caused not only overdiagnosis of Bipolar-II in general, but it was misinterpreted to mean that activation, a common adverse reaction to antidepressants, was diagnostic for bipolar disorder.
It is not.

Road to hell is paved with good intentions & bad unintended consequences.
We included Bipolar-II in DSM-IV to reduce risk of antidepressants causing mania/rapid cycling in people with bipolar risk.
But Pharma promoted massive over-diagnosis & overuse of harmful antipsychotics.

Article specifies PSYCHOTHERAPY, not the reflexive throwing of antidepressants at women that usually results from screening, which is known to produce many false positives.
Of course "depressed" mothers affect offspring -- often this is in a familial context of financial strain.
Many research studies have shown that untreated depression during pregnancy adversely impacts the mental health of the child as well as the mother. ow.ly/R4wK50ZZ4oE

What would a good country do for people with severe mental illness:
1)Build affordable housing, not data centers
2)Spend $$s on treatment, not jails
3)24 hr crisis teams, not cops
4)Community mental health centers, not waiting lists
5)Clozapine/lithium clinics, not neglect
This is the sort of misinformation that gets people addicted.
REGARDLESS OF DIAGNOSIS, if you take Ritalin often enough, you will become physically dependent on it. If you have the urge to take Ritalin frequently, you may meet the criteria for addiction, REGARDLESS OF DIAGNOSIS.

Core concept: Why Benzodiazepine Withdrawal Varies in Presentation and Recovery
Dose and taper rate matter, but they do not fully explain why symptom intensity and stability vary between people—or within the same person over time.
A dose reduction does not occur in isolation. Its effect may depend partly on the demands the nervous system is already managing and the capacity available to manage them.
This article introduces four concepts for understanding that relationship:
• Signal load
• Regulatory range
• Physiologic reserve
• Reinforcing loops
Together, these concepts offer a broader way to understand a person’s current clinical state and what stabilization is trying to accomplish.

Great News:
1)Benzo use fell rapidly last decade (7.5% to 5.5%)
2)Young adults 45% decline
3)Prescriptions fell most among:
Pain specialists 63%
Primary care MDs 44%
Psychiatrists 38%
Remaining Bad News
1)Still way too high
1)Senior rate 10.7%
2)Nurse practitioner rate doubled
Here's a first... benzodiazepine prescriptions declined in the past decade.
Lower rates among all clinicians except one group - that I suspect is due to workforce shifts:

We killed 92 of 94 new diagnoses suggested for DSM-IV.
The 2 we added (Aspergers/Bipolar 2) caused more harm than benefit.
I suggest no new disgnoses until we have solid biological tests.
Dumb new labels take on a dangerous life of their own
(see great description below).
Sometime before 2016, a psychiatrist started asking their late-diagnosed adult patients with ADHD whether they thought they were more sensitive to rejection and failure than other people.
They said that they were. He didn’t (as far as I can tell) do any comparison with people with mental health problems, or with those with social anxiety. He didn’t look at children or those diagnosed in childhood. He just asked his patients to self-compare with others.
There were no attempts to test out whether this was specific to ADHD, nor universal in those with ADHD.
In 2016, the psychiatrist, William Dodson, gave this a name - rejection sensitivity dysphoria - and started writing about it. It resonated with influencers who created content talking about their RSD. It quickly multiplied. Articles were written about it on official sounding websites. Qualitative research was done, asking people who had heard about RSD whether they experienced it and what it was like.
By 2024, Dodson was saying that RSD was neurological and genetic, not related to trauma and wouldn’t respond to psychotherapy.
By 2026, bestselling author Alex Partridge wrote a whole book about his RSD, pinning it squarely to ADHD and saying that RSD ‘explained my entire life’. He gave a convincing sounding brain-based explanation about ‘glitches’ in how the ADHD brain processes dopamine and nor-epinephrine, and how these lead to rejection feeling far more brutal than for people without ADHD. There is no evidence of this that I have found.
And in this way a psychological and emotional reaction was framed as a neurobiological symptom. Millions started to think that perhaps they had ADHD, because they were sensitive to rejection and failure. RSD spread, and soon people started saying it was part of autism too.
Why does that matter? Because when people think something is due to how their brains work, they think that it is set in stone and won’t change. They think that medication is their only option, and that psychotherapy has nothing to offer. Emotional sensitivity is a really common psychological symptom, and it hasn’t been shown that it’s uniquely part of ADHD. RSD is an example of how neurodivergence is expanding to incorporate emotional reactions, and in the process turning psychology into neurology.
With RSD, the most basic scientific questions haven’t been answered, but you’d never know that from what you see online. There, it all seems so certain and clear.
It’s not.

Psych medicine is necessary in treating severe mental ilnness- but rarely sufficient.
Equally important to treat the person & context.
People with severe psych problems almost always also need help with family, social,
psychological, financial, &/or housing issues.
And hope!
If the goal is to set up medications for serious mental illness to succeed, they should be prescribed not in isolation but as part of a biopsychosocial package. 🔗👇

When symptoms involving sleep, digestion, movement, arousal, and other functions appear during the same period, it can feel as though many separate things are going wrong.
But the body does not manage these functions in isolation. Its regulatory systems have different roles, but they interact.
This does not mean that every symptom has the same cause. It means that a cross-system symptom picture is biologically plausible.
New or changing symptoms still deserve appropriate medical evaluation.

Rethinking Psychiatric “Overdiagnosis” as a Response to Clinical Need: The Diagnostic Stretching Hypothesis
“The intention of my hypothesis is to offer an explanation for both why diagnoses have increased (this is in response to genuine clinical need and the population burden of distress/impairment) and why this invites skepticism (stretching beyond official thresholds is problematic for various reasons and is met with understandable suspicion).”

Very few people escape some period of misery in their lives. By "stretching", psychiatry sees "clinical need" in close to 100% of the population.
Problem is that in psychiatry, "clinical attention" = invasive treatment with drugs. "Stretching" is an outcome of drug marketing.
