1/15 🤔Does end-stage renal disease (ESRD) make diabetes disappear? No, but in some patients with ESRD, insulin needs fall. A1c drops. Diabetes medications can be stopped. This has been called “burnt-out diabetes.” Let's examine why.

One of my missions is to make Peripheral Neuropathies more understandable for non-experts. Recognizing that physicians often find PNs challenging, especially in the hospital setting, I collaborated with @MayoClinicNeuro Neuromuscular hospitalists, @ReeceHass and @SantilliAshley, and my former NM co-fellow #JMartinezThompson to develop an engaging infographic that simplifies how to approach neuropathies leading to hospitalization. The incredible artistic skills and creativity of @ReeceHass truly brought this project to life, exceeding all expectations. We hope you find this helpful. neurology.org/doi/pdf/10.121…

Some of the best advice I ever received was that, once you get beyond medical school, most of your learning is self-directed. People will teach you. They’ll answer questions, supervise you, and create opportunities for you to learn. But by the time you’re an intern, resident, or fellow, there’s an expectation that you’ll do your part to seek out the experiences and knowledge you need. There won’t always be formal lectures or someone telling you exactly what to read next. A lot of learning comes from paying attention, asking questions, reading on your own, and making the most of the patients you take care of. I remember a fellow apologizing because they needed to miss a day on service. They were worried it would leave me shorthanded. I told them something I’ve said to other trainees over the years: I don’t need you to be there. I mean that in the kindest possible way. I can manage the service and take care of the patients. The reason I want you there is because you’re learning. Every patient, every family meeting, every procedure, every conversation with a consultant, every note you write is part of becoming the physician you’ll eventually be. My role is to help create that environment, but I can’t do the learning for you. I think that’s one of the biggest transitions in medical training. At some point, your education becomes less about what someone teaches you and more about what you choose to learn from the opportunities in front of you.

I knew that bactrim “falsely” increased serum creatinine (Cr) without affecting kidney function, but today I learned it’s because trimethoprim is a cation. It competes with Cr for the cation transporters in the proximal tubule which causes decreased Cr secretion into the urine.

Proud of the performance by Glass on the recent ARISE research network's Medical AI Superintelligence Test. Glass outperforms other medical AIs and frontier models across many benchmarks and ranks #1 on the test of complex New England Journal of Medicine cases.
Glass 5.6 Max ranks #1 on CPC-Bench for medical diagnosis in the Harvard-Stanford ARISE Medical AI Superintelligence Test (MAST) - ahead of systems from AMBOSS, OpenAI, Doximity, Anthropic, and Google. glass.health

We are releasing free AI tumor board by @MeDucationai Paste a de-identified oncology case and nine specialist AI agents will research it against live PubMed and ClinicalTrials.gov, argue with each other, and hand you a citation grounded consensus in about five minutes. It is free and there is no account. meducationai.com/ai-tools/tu… The panel convenes based on the case. Medical oncology, heme, rad onc, surg onc, molecular, pathology, transplant and cellular therapy, trial matching. The clinical pharmacist is always on as a safety gate, which was a deliberate choice: interactions and dosing are where an LLM quietly hurts someone. Two design decisions I care about more than the agent count. First, every clinical claim has to carry a citation from a live source, so fabricated references are structurally impossible rather than merely discouraged. Second, a judge scores consensus and opens another round when the panel disagrees, so you see the disagreement instead of a smoothed over answer. Cases run in memory and expire within thirty minutes. No accounts, no storage. Still, do not paste identifiers. Synthetic and educational cases only. This is a free research preview and it is for research and education, not medical advice. It does not replace your institutional tumor board and it is not reviewed by a physician. If you run a case, I want to know where it was wrong. That is more useful to me than knowing where it was right.

At leading AI companies, software agents are using AI to write 90% of their code. The same thing will happen in medicine, where doctors will use AI to complete 90% of clinical work. In some cases, the AI-Native doctor is already here.

I am uncomfortable with the proliferation of AI-generated radiology images in social media education. They're not labeled as AI and undiscerning viewers are routinely fooled into thinking they're real.

I’ve been using Pearls regularly, and feel it’s now ready for a broader release, so I made a small video :) 🔵 Pearls is an app for clinicians I built for capturing clinical pearls while rounding or studying. Some useful features: - Automatically fetch context: after you add a concept or phrase, an AI agent will (optionally) fetch additional information and augment your note. I like pasting clinical trials, and letting Pearls update my note by fetching more details in the background. - Auto generate Anki cards: you can convert any Pearl into an Anki card and directly import to your deck. - Sync across mobile/web devices, auto-tagging, search and filtering Pearls I’ve been using it quite regularly and have found it valuable in helping me capture and remember my own clinical knowledge. Available for free: contextualmed.com/pearls/
