🚨 My new paper on small fiber #neuropathy in NONPAINFUL conditions is published. Neuropathy is traditionally defined by neuropathic (tingling, numbness, burning, shooting) pain, but there are neurologic and non-neurologic conditions where objective findings of small fiber neuropathy are highly prevalent despite the pain being absent or minimal. I outline comprehensive treatment options for small fiber neuropathy with both evidence-based and emerging therapies in the Table. 🧠 clinmedres.org/content/24/3/…

In new study, we show that SARS-CoV-2 has evolved decreased inherent antigenicity in humans, as quantified by a ~3-fold decrease in the number of naïve human B cells that can bind the spike RBD. doi.org/10.64898/2026.10.15.…

SARS-CoV-2 community spread in Aotearoa NZ began with Omicron in January 2022 (the 2021-22 data point on this chart). ED presentations jumped up, and they kept increasing (even though the SARS-2 waves got smaller after mid-22). This is a growing burden of SARS-2 chronic harm.
The latest report from the college of emergency department doctors shows record-high ED presentations in New Zealand - more than 1.4 million annually, up 21 percent from 2015-16. That's compared to population growth in the same period of 13 percent. 1/

"Korea makes COVID-19 vaccination part of routine national immunization program" KDCA says COVID-19 continues producing variants that can evade immunity, while people 65+ account for 63% of hospitalisations. It is shifting to routine vaccination to protect high-risk groups.

A slightly more personal post than usual. I’ve always believed that scientific disagreement can be discussed without turning into insults, mockery or personal attacks. Over the last few days, I’ve realised that this seems to have become much harder in some parts of this community. And honestly, I find that sad. (1/9) 🧵

The @ISLCPAIS conference website was hacked more than twice including during the event. We received massive amounts of criticism online, and more than once @miketweetchap received threats at his home address, simply for trying to bring people together. Michael, you did nothing wrong. You poured your heart and soul into this conference, while carrying the additional reality of being a Long Covid patient yourself. I saw how much work, energy and care you put into making this happen, and I honestly think what you accomplished was tremendous. Those of us working in this field are fighting on so many fronts. We are fighting #COVID and #LongCovid denial and dismissal, while also having to deal with radical voices within our own community attacking the very people who are trying to move the field forward. It is exhausting. But for the last days, we saw what can happen when people come together despite all of that. Scientists, physicians and patient advocates in the same room, sharing ideas, building collaborations and trying to find better answers for patients. So please don't let the attacks make you question why you did this. You did it because you care. And you created something that brought people together when we desperately need more of that. I’m incredibly grateful to be part of this and I hope you take a moment to be proud of what you accomplished. ❤️
I have received criticism and even a threat at my home address for organizing the @ISLCPAIS conference. I pored my heart and soul into it because I believe boosting collaboration and bringing together the bright minds is fundamental for speeding up the process. What did I do wrong? It’s as genuine question after self reflection…

I do believe science can unite safely & accomplish 5 months worth of time into 4 days. Whatever it takes to get this subject matter to the next generation of scientists & doctors who have been encouraged to study #LongCovid. Ideas are priceless.

#ISLCPAIS2026 abstract review. My thoughts on abstract 2 One of my fav studies which basically replicates @VirusesImmunity bigger studies in transfer of IgG from patients with #longcovid to mice. This study provides further evidence for an immune-mediated mechanism of pain in Long COVID, showing that patient-derived IgG targets peripheral sensory neurons in vivo and ex vivo. These findings strengthen the rationale for testing immunomodulatory approaches such as IVIG in carefully designed clinical trials. Importantly, funding strategies should not focus exclusively on viral persistence: the growing evidence for pathogenic autoantibodies and immune-mediated neuronal dysfunction makes immunomodulation a parallel, high-priority therapeutic avenue that warrants properly powered clinical trials. I’m surprised this is still not on the priority list

Important review about #POTS from my colleague @satish_r_raj, who is part of our @ISLC_PAIS society and presenting in our meeting this Friday. #LongCovid
#PosturalOrthostaticTachycardiaSyndrome (POTS) is a chronic autonomic disorder defined by orthostatic symptoms with a sustained heart rate increase within 10 minutes of standing or head-up tilt, without orthostatic hypotension. 🔗 ja.ma/4zzP3Kn

Only one week to go until the 2026 Amsterdam ISLC-PAIS Conference brings together more than 300 clinicians, scientists, early-career researchers and patient advocates, all committed to advancing our understanding of Long COVID and other Post-Acute Infection Syndromes. With more than 100 scientific sessions, the conference will cover the latest research, clinical insights and developments across the field. To make the key takeaways accessible beyond the conference itself, we are organising the Patient Recap Livestream Session. A dedicated session that translates the most relevant conference insights into clear and accessible information for patients. islc-pais.org/program/patien…

Outside a Sydney emergency department this week. Perhaps the penny has dropped that staff keep falling ill and going off sick, and mitigation is possible. Surprising % of patients and staff complying, too.
