🧬 Day 8 of 15 | Sugar & Your Liver
Here is what happens inside your liver every time you drink a sugary beverage — step by step:
1️⃣ Sugar enters your bloodstream
2️⃣ Fructose is sent DIRECTLY to the liver — unlike glucose, it bypasses all other tissues
3️⃣ The liver converts excess fructose into triglycerides (fat)
4️⃣ Fat accumulates inside liver cells → Fatty Liver (MASLD)
5️⃣ Fat triggers chronic inflammation → MASH
6️⃣ Inflammation activates hepatic stellate cells
7️⃣ Stellate cells produce collagen → FIBROSIS begins
8️⃣ Repeated injury → scar tissue replaces healthy liver → CIRRHOSIS

In this fantastic paper, the authors evaluate therapeutic plasma exchange (PLEX) in alcoholic hepatitis + ACLF.
📊 Post-PLEX, MELD declined significantly from 31.6 to 26.5, p<0.001, with greater reduction among survivors.
🧬 Post-PLEX AARC score predicted 1-month mortality (AUC) 0.808; p=0.002).
🏥 No major adverse events occurred.
PLEX is a safe, feasible, and potentially life-saving in severe alcohol-associated hepatitis–related ACLF — particularly for patients ineligible for steroids.
