Pragmatic Trials: the evolving landscape
#kidneywk
📍 Historically, nephrology trials have been small and highly selective: 66% were single-center and 65% enrolled fewer than 100 participants. Traditional trials often operate separately from routine care, limiting generalizability and increasing complexity

⏩First trial in today’s LBCT session #kidneywk
📍 ATTACK Trial | Aspirin for primary cardiovascular prevention in CKD
👉CKD increases cardiovascular risk, but whether aspirin offers a net benefit for primary prevention remains uncertain
👉In the general population, aspirin modestly reduces major vascular events (11% relative reduction) but increases major bleeding (43%), with little absolute net benefit
👉ATTACK is a pragmatic, multicentre, open-label RCT comparing aspirin 75 mg daily with usual care in adults with CKD stages 1–4, without established CV disease or increased bleeding risk
