Saluting and sending love to the ostomy warriors and the great ostomates all over the world. Every journey with a stoma isn’t the same, but everybody with an ostomy story deserves respect, and not to feel any shame.

Doctors manage risk every day but its the patients who are at risk and trust doctors with that responsibility
Increasingly doctors are responsible for the risk to patients of colleagues and systems over which they have no control and for which there is no evidence of safety
@CMO_England was an advocate for PAs and he failed to understand the safety concerns of physicians prior to the RCP EGM
The NHS 10 year plan and the skills escalator in particular loads doctors with responsibility for the risks patients will take for taskification and doctor substitution
Chris Whitty: Doctors must be trained in managing risk in patient care, CMO says.
The BMJ and @acmedsci inaugural joint annual lecture given by Prof. Whitty @CMO_England covered risk, uncertainty, and medical decision-making.

Reading the Thirlwall report. Leadership behaviours putting organisational reputation above patient safety is a strong theme (as expected). The big recommendation is regulation of NHS managers & duty of candour at an individual level.
I think both are overdue and welcome.

Turns out the biggest suppressor of whistleblowers in the NHS is the NHS itself.
🚨 EXC: NHS England has suppressed the work of investigators carrying out dozens of national clinical audits. NHSE has stopped investigators alerting media, watered down recommendations and limited the way their reports are written, published and used: thetimes.com/uk/healthcare/a…

Torpenhow Hill, England
When the Saxons arrived and asked the locals what that hill was called, the Welsh replied, “Pen,” which simply means hill in Welsh.
The Saxons, apparently not spotting they’d already been given the full answer, added their own word for hill: tor. So it became Torpen - literally Hill Hill.
A few centuries later, the Norse turned up and repeated the exercise, adding haugr, their word for hill. Now it was effectively Torpenhaugr - Hill Hill Hill.
Then the English arrived, looked at the whole thing, shrugged, and added “Hill” on the end for good measure.
Thus we ended up with Torpenhow Hill:
Hill Hill Hill Hill.
It’s comforting to know that for over a thousand years, successive generations have been independently deciding that the previous lot hadn’t made it quite clear enough that it was, in fact, a hill.
Language is magnificent.
Humans… less so.🤷🏻♂️🤣

🚨 Hundreds of NHS staff a year are being investigated for sexual misconduct with many allowed back to work even after being found guilty.
Now Britain's top surgeon says enough is enough and a radical overhaul is needed to protect staff & patients:
