GLP-1 drugs in #IBD are a hot talking point at the moment - and they're throwing up some interesting findings, to boot.
We've just had a VERY special expert guest confirmed for our podcast hot seat on this one! 🤩 So please do send us your questions on this topic. Thanks.

Is proximal small bowel Crohn worse than distal bcs the Tx is less effective there?
Well, No...
In our @sheba_medical #capsule study now published in @AGA_CGH , biologics induced similar mucosal healing in proximal vs distal SB.

🎯 STRIDE-III is here!
The new treat-to-target framework for #IBD, presented at #UEGWeek2026 and published in @LancetGastroHep.
🔑 The big shift: not every target asks for the same thing.
🔴 Actionable → miss it, change treatment
🟡 Adjunctive (NEW) → histology in UC, transmural healing in CD, urgency: optimise & monitor
🟢 Patient-centred → fatigue, disability, QoL: shared decisions
📊 1,018 studies · modified Delphi · ≥75% agreement

🎯 Should we treat UC all the way to histology? VERDICT says... maybe not!
📍 672 adults with moderate–severe UC on vedolizumab, randomised to 3 treat-to-target strategies (#UEGWeek2026 late-breaker)
🔬 Reaching disease clearance (symptoms + endoscopy + histology) → 67% lower risk of UC complications vs a symptom-only target
📉 HR 0.33 (95% CI 0.17–0.67), p=0.0014
🤔 Endoscopic target vs full clearance: no difference (HR 1.10)
⚠️ Analysis only includes patients who reached their target (59.5%)

Pleased to say that Nigel & I now have a confirmed recording date for our fatigue in IBD podcast episode. Sorry this one took so long!
Then we're off on a virtual visit to the US to record the next trio: GLP-1s in IBD (massive interest right now); pain management; and IBD Qorus.

A welcome step forward in Crohn’s disease care!
I am excited about the FDA approval of subcutaneous induction with risankizumab (SKYRIZI) for Crohn’s disease.
Having cared for patients with IBD for many years, I have seen how the burden of treatment extends well beyond the disease itself. Traveling to infusion centers, taking time away from work, arranging transportation, and balancing treatment with everyday life can be challenging.
The ability to initiate treatment at home offers patients something valuable: greater flexibility, convenience, and potentially more control over their treatment journey.
The encouraging clinical and endoscopic outcomes from the AFFIRM trial make this development even more meaningful.
Of course, convenience must be accompanied by efficacy, safety, affordability, and access.
I see this as another positive step toward making effective IBD care fit better into patients’ lives, rather than asking patients to continually rearrange their lives around treatment.
Innovation in IBD should not only improve outcomes. It should also improve the patient experience.
