Incredibly, hospitals are now paying for AI companies to disrupt them. 🤦‍♂️ Patients linking their MyChart accounts to @OpenAI's ChatGPT rose more than 7,000% in a single month at 69-hospital @AdvocateHealth, its CIO Bobbie Byrne told @Becker’sHR. Patients set this up themselves in ChatGPT, so that the AI can explain their lab results, scans, and doctors’ notes. You know, instead of one of Advocate's doctors. To add insult to disruption, every time a patient asks ChatGPT a question, Advocate’s Epic system has to look up and send the records and pay for the computing resources that make that happen. AND — brace yourself — Federal law says Advocate can't charge patients, or their apps, for that access to the patient's own records. So a health system is paying for a competitor’s access to its own patients’ records. The competitor takes away the followup visit, and the healthcare system pays for that to happen — both in lost revenue and in increased computing costs. That’s Interpretation Migration, subsidized by the disruptee. buff.ly/DlyODMb #DrYou #InterpretationMigration #Disruption

Looking forward to this. #AHA2026
Most clinicians & scientists don’t set out to start a company. They set out to solve a problem. At #ScientificSession26, our first-ever Innovation Forum with @AHAScience opens with The Entrepreneur’s Journey: a candid talk on getting from idea to impact. professional.heart.org/en/me…

This is the way. Thanks @hmkyale for showing the world what good like.
We don't ask whether a model touched their prose. We ask authors to own every word. @JACCJournals parted w/ICMJE & doesn't require disclosure of AI writing help: the rule is unenforceable, biased, & not good for medical communication. My Editor's Page: jacc.org/doi/10.1016/j.jacc.…

We can debate about how LLMs are going to replace books and brains, but this is still one of the greatest medical pocketbooks ever made. You just can't thumb through an LLM. Out of print.

The best way to limit AI slop in presentations and compel authenticity at scientific and clinical meetings is to eliminate slides entirely for most topics. If I want a comprehensive overview, I’ll ask an LLM rather than sit through 80 slides. If I want your insight, then I’ll come listen. PowerPoint has occupied only a small sliver of time in the history of medicine. #noslides
AI fatigue at conf is already here. I see presentations after presentations using the same AI format. No flaws but look the same. I cannot recall the speaker behind any of them. Other than occasional graphics, I am sticking to my style even if imperfect!! @drjohnm @SVRaoMD

In 1980, 21% of NIH-funded principal investigators were under age 35. By 2015, this proportion was 3%. The dramatic ageing of independent scientists is bad because old people are simply worse at science than young people. Latest meta-scientific deep-dive from me 🧵 ⬇️

My grandpa’s name was Dan Singh. His father, Katha Singh, came to America from India decades ago to seek a better life. He became a raisin farmer in California. My grandpa eventually took over the family business and also worked as a truck driver. This post is racist and demeaning to anyone with the last name Singh.

A terrific choice to lead @nyulangone through its next chapter. @SVRaoMD is unique — peerless in many ways — and will do great things and bring the program to new heights.
We’re pleased to announce that @SVRaoMD has been named director of the Leon H. Charney Division of Cardiology, where he’ll oversee clinical care, research, and education while advancing innovation in cardiovascular medicine. Learn more about Dr. Rao: bit.ly/3SAav1t

Not a great question. Verapamil-sensitive VT (or Belhassen's VT) is a clinical entity referring to a re-entrant fascicular VT with part of the circuit that is sensitive to calcium channel blockade. While verapamil isn't the right choice in poly VT, there are instances where it is the right drug.
Which of the following drugs should NOT be used in the management of ventricular tachycardia ? A) Amiodarone B) Lidocaine C) Verapamil D) Procainamide

Congrats to @gregorymmarcus and his team. Greg is unique in cardiovascular science: time after time, Greg does the studies that are hard but sound, challenge dogma, and others aren’t able or willing to do.
Contrary to our hypothesis, we found in a randomized crossover trial of smoked/ vaped #cannabis, that exposure to the drug led to LESS cardiac ectopy (mainly driven by less premature atrial contractions, an important predictor of #afib); no differences in sleep, step counts or glucose using wearable sensors. @DrDave01 @leftbundle @drjohnm @PrashSanders @ethanjweiss @NguyenHEARTYlab @UCSFCardiology @UCSF @DebbeMcCall @stopafib @KnowYourPulse Congrats to first author @Adielias5 Full text link: authors.elsevier.com/a/1nbTn…

These are rational choices in academic medicine when RVUs pay the way. The real disparity is that clinician scientists can only do science if they can afford to — independent wealth, high spousal income, or self sacrifice. The system must change.
Replying to @salimhayek
I've dropped 90% of my academic work, will be giving my last lectures at major international conferences soon, resigned from editorial committees, have stopped reviewing papers, and will never be the lead author on another paper. Same salary, same benefits, more free time.

Often, trial results are taken as gospel when in fact, they are blunt instruments designed to detect high signal over short time periods. While obs studies often overestimate, a negative RCT doesn’t always translate to evidence of absence. There can still be absence of evidence.
🫀 Why do observational studies say exercise markedly slashes CV events and death, but RCTs often don't? 🤔 Reverse causation + confounding inflate cohorts? or Shorter, poor adherence RCTs miss real effects? The truth: Real and large, real just smaller, or not real? 🏃📊
