We need to rethink our diagnostic approaches and move quickly early recognition is key!".
🫀From Healthy Atria to Atrial Failure: Rethinking Atrial Cardiomyopathy (AtCM) from the #HFA_ESC clinical consensus statement:
📍AtCM extends beyond #AF: Electrical, mechanical & structural abnormalities may precede clinically apparent AF or #HeartFailure
📍Diagnosis: Electrical atrial dysfunction (P-wave score ≥1) PLUS ≥1 of: mechanical dysfunction, atrial enlargement or excessive atrial fibrosis
📍Proposed diagnostic thresholds:
- P-wave ≥120 ms (standard ECG) or ≥150 ms (amplified ECG)
- Advanced interatrial block: P-wave ≥120 ms + biphasic morphology in ≥2 inferior leads
- LA reservoir strain <23%
- LAVi >40 mL/m² (>48 mL/m² in women >65 years)
- Atrial fibrosis: thresholds pending validation
📍New P-wave scoring system (0–4): Ranges from normal P-wave (0), prolonged P-wave (1), advanced interatrial block without (2) or with paroxysmal arrhythmia (3), to persistent atrial arrhythmia (4).
📍Atrial failure: End-stage AtCM, characterized by persistent AF or HF with symptoms attributable to atrial disease.
📍Early recognition matters! Risk factor & comorbidity management may help prevent or delay progression to atrial failure.

So the final analysis for the pivot pivotal preservative is in, and it looks like adjuvant pembrolizumab shows promise for high-risk ccRCC patients!.
Final analysis of the pivotal KEYNOTE-564 trial in #KidneyCancer presented by @DrChoueiri at #ESMO26. After 7 years of follow-up, adjuvant pembrolizumab continued to extend disease-free survival and overall survival in high-risk clear-cell renal cell carcinoma.

📰Just in: T-cell–redirecting therapies continue to demonstrate improvements in early-relapse myeloma.
Topline data from the phase 3 MagnetisMM-32 trial showed elranatamab improved PFS vs SOC in R/R multiple myeloma treated with at least 1 prior line of therapy that included an anti-CD38 monoclonal antibody and lenalidomide #mmsm #oncology

Should we test for Alzheimer's disease before symptoms begin? What resonates with me about this thoughtful JAMA Viewpoint by neurologist Gayatri Devi is the important reminder that just because we can test for Alzheimer's disease does not always mean we should. We are entering an exciting era when blood tests can detect Alzheimer's-related proteins years before memory problems appear, but what do we tell someone who tests positive and may never develop dementia? The article highlights a striking finding: in adults younger than 60 without symptoms, the positive predictive value of one blood biomarker test for detecting amyloid pathology dropped to just 38%. A positive test could create enormous anxiety, affect long-term care or life insurance, and turn an otherwise healthy person into what the author calls a patient-in-waiting. I believe strongly in early detection, and I am excited about the possibility of preventing Alzheimer's before symptoms begin, but we must distinguish detecting abnormal proteins from predicting who will actually develop dementia. Until we have better tools to predict individual risk and proven preventive treatments, we should be cautious about routine testing of folks without symptoms. In the meantime, we already have meaningful actions we can recommend, including exercise, better sleep, controlling blood pressure, addressing hearing loss and staying socially engaged. The goal of medicine should not simply be to identify disease earlier, but to help folks live longer, healthier and better lives.

Belaid et al report the largest single-country cohort of RAG1/2 deficiency (n=128, Algeria). 2 founder variants explain 77% of cases, yet identical genotypes produced 3 different phenotypes. Mortality (89%) reflected transplant access, not diagnostic delay hubs.la/Q04Cjrmw0
Uncertainty around these products' sources, contamination risk, and potential displacement of approved treatments are all concerns that clinicians should address with patients.
💬 Viewpoint by Sophia Samantaroy, MS, Isabella Xu, MPH, and Peter Lurie, MD, MPH: Proposed expansion of access to compounded #Peptides may have implications for patient safety and clinical practice because these products are not subject to the same requirements as FDA-approved drugs.
Clinicians may see increasing patient demand, should ask about peptide use, and should counsel patients about uncertain sources, contamination risk, and displacement of approved treatments.

Totalmente. El examen necesita un cambio ya. No puede seguir siendo tan subespecializado porque al final da más peso al azar. Debería ser un temario cerrado y de medicina general.
The monitoring protocols are pretty comprehensive - they're checking all sorts of biomarkers, joint assessment, and even doing CT scans!.
