🚀 Exciting work by @brian_rini and colleagues! IRIS-RCC brings us closer to reproducible, individual-sample molecular subtyping in RCC. We are exploring a complementary approach in mRCC: vascular architecture as a biomarker of IO sensitivity, assessed on routine H&E or CD31 IHC. Our initial data suggest that low vessel branching may enrich for IO response, while our vascular score has also shown baseline prognostic value in CheckMate 914. The exciting opportunity: a potentially scalable, pathology-based biomarker that could be easier to integrate into routine clinical workflows. Next step: validation in digitized H&E from large, randomized mRCC cohorts—critically, to distinguish predictive from prognostic value. Our preprint 👇 medrxiv.org/content/10.1101/… @OncoAlert @weoncologists @DrChoueiri
Several groups have attempted to predict the IM151 RCC clusters with flawed methods. We developed IRIS-RCC (Individual RNA-seq Intrinsic Subtyping for RCC; available at irisrcc.vindhyadatascience.c…), to predict clusters using an individual-sample model. nature.com/articles/s41698-0…

ESMO 2026 | A cautionary tale for immunotherapy de-escalation! MOIO Trial (LBA10) – Phase III non-inferiority trial testing 3-monthly immunotherapy vs standard dosing in responding patients with advanced/metastatic cancer. 📊 Key results (N=308): • Median PFS: 21.9 vs 25 months (reduced vs standard; HR 1.35) • Risk of death: HR 2.0 (95% CI 1.1–3.9; p=0.0143) ⚠️ • PFS HR in urothelial cancer: 2.9 • Grade 3–4 immune-related AEs: 9.5% vs 10.5% ❌ Non-inferiority NOT demonstrated. ❌ No meaningful safety advantage. ⚠️ Signal of inferior survival with reduced-frequency IO. 💡 Take-home: Less frequent immunotherapy is not necessarily equally effective! Important: These are interim results with relatively small numbers and heterogeneous tumour types. #ESMO26 #MOIO #Immunotherapy #Oncology @OncoAlert @myESMO

Hear ye hear ye!! New pembrolizumab TMB tumor-agnostic threshold is 13 rather than 10. 🔥 Our data suggests it is 16 mut/mb (best threshold) Data coming soon! Responses can be phenomenal. I have patients who went from “next step hospice” to “CR ongoing for years— aka cured”

Putting STAMPEDE, ENZARAD, PROTEUS & DASL into context with the latest #ESMO26 updates: STAMPEDE: Abiraterone-based intensification improved MFS AND OS, predominantly with RT. New #ESMO26 Decipher data suggests differential abi benefit in cN0 disease (interaction P=.01); using this to omit treatment still requires validation. ENZARAD: At 8 years, MFS was 74% vs 72% (HR 0.88; P=.34). No significant overall MFS benefit from enza + ADT/RT. The cN1/planned pelvic RT subgroup signals remain hypothesis-generating. DASL: new at #ESMO26: 1,106 patients; 96 weeks of daro v. placebo + ADT + prostate/bed AND pelvic RT. Four-year conventional-imaging MFS: 95% vs 88% (HR 0.42). Very-high-risk population, including selected postoperative patients. Interim analysis; OS immature. PROTEUS main trial: Perioperative apa + ADT improved 5-year PSMA-inclusive MFS versus ADT alone: 78.2% vs 73.5% (HR 0.80). Conventional-imaging MFS was not statistically significant. #ESMO26 post hoc PSA-response analysis adds prognostic information. PROTEUS randomized substudy: new at #ESMO26: Perioperative apa/ADT + RP/PLND (n=102) v. immediate RP/PLND (n=306). At 37.8 months’ follow-up: • 3-year EFS: 63.0% vs 41.4%; HR 0.38. • Secondary PSMA-inclusive MFS: HR 0.49; nominal P=.0193. • Subsequent postoperative RT: 20% vs 43%. The substudy strengthens a "perioperative strategy". But conventional-imaging entry followed by PSMA-inclusive endpoints matters. PSA-based EFS, conventional MFS and PSMA-inclusive MFS are different endpoints. These trials support specific intensification strategies; they cannot rank the drugs or establish RP-versus-RT superiority. The decision remains who benefits enough to justify the ENTIRE treatment burden.

#ESMO26 @myESMO Presidential session presentation by session @ProfGLongMIA INTerpath-001: intismeran + pembro vs pembro in resected stage IIB–IV melanoma (n=1,137) RFS HR 0.58, 2-yr RFS 82% vs 70%
DMFS HR 0.62, 2-yr DMFS 89% vs 80% 1st phase 3 win for an individualized cancer vaccine against an active IO control. OS is pending, but the adjuvant melanoma bar just moved! Moving moment as @ProfGLongMIA paid tribute to Dr. Jeff Weber, who designed this trial and enrolled patients until his passing. Jeff was a dear friend and my colleague at @MoffittNews and today's results are his legacy 💙 Bladder next: INTerpath-005 in MIUC, look forward to seeing the data in future!

Darolutamide with ADT and locoregional radiation therapy (RT) in high-risk prostate cancer (DASL) showed Darolutamide reduced the risk of relapse and time to CRPC. Tolerably was good. It’s imporant to identity which patients benefit, as mortality is low, and whether latest staging with PSMA-PET changes things #ESMO26 @Silke_Gillessen @ChrisSweens1

VOLGA also explored short periods of EV with duvalumab and tremelimumab in muscle invasive bladder cancer #ESMO26 @NEJM. Results showed EVDT improved EFS vs cystectomy (HR 0.61). Significant OS was not reached at this interim (HR 0.77 -ongoing analysis). EVDT = 10% higher rates of G3/4 toxicity and less EV delivery than the EV durvalumab arm. More is not always better. It appears the addition of treme partly compromised delivery of EV, which likely accounts for the more modest results compared to the doublet @OncoAlert

ESMO 2026 - presidential LBA | VOLGA Phase III – Perioperative Durvalumab + Enfortumab Vedotin (EV) in cisplatin-ineligible/refusing patients: 📊 EV + Durva vs Control • 2-year EFS: 64% vs 45% (HR 0.51; p<0.0001) • 2-year OS: 78% vs 69% (HR 0.58; p=0.0027) • pCR: 45% vs 10% Adding tremelimumab improved EFS (HR 0.61) but no significant OS benefit yet, with higher grade 3/4 toxicity (42% vs 30%). 🔥can this stop the need of post op EV ? Probably yes . New SOC 🙂👍 Looking forward the full datasets @OncBrothers @OncoAlert @myESMO #ESMO26 #VOLGA #BladderCancer

It hard to explain why 1 trial with gem/cis +/-PD(L)1 (NIAGARA) is +ve for EFS/OS and 2 very similar trials (presented today) are negative in MIBC. Potentially NIAGARA has the highest risk population, not all respond to IO and the maintenance phase the most effect- but the OS of gem/cis pembro HR of 1 is concerning. This reflects what we saw in metastatic disease with contradictory results for the big combination trials. Overall EVP looks better and more consistent #ESMO26 @myESMO @Annals_Oncology @OncoAlert

I’m so sick of reviewers using ChatGPT to review papers. It’s atrocious having to respond to the same garbage from all 3 reviewers where each comment requests 5 new analyses. Just don’t agree to review papers if it’s too much work 🤷‍♂️ Even if you used AI to review my paper- at least ensure the asks are genuine and then choose the most pertinent/relevant comments to include in your review? Is that too much work? I’m sure journal editors recognize that this is a real problem but can’t do much to rectify this issue.
