Important translational research using multi-omics to explore payload resistance, likely a major driver of ADC resistance.
In this ADC-rich era, could switching to an ADC with a different payload allow us to later rechallenge with the original payload?
#ESMO26
Honored to have the opportunity to present the results from our translational #CHARISMA study, unveiling mechanisms of resistance to TOPO1 ADC. A true team work involving the amazing @DFCI_BreastOnc research teams and @AstraZeneca Translational Medicine. 🧬 #ESMO26
HER2+ MBC with CR/PR ≥2 years, ctDNA monitored every 3 months.
81% remained ctDNA-negative without radiographic PD.
In all patients with baseline radiographic RD who progressed, ctDNA positivity preceded or coincided with PD.
🌅The era of ctDNA-guided trials is here.

HER2-CR study of MRD in long-term responders to anti HER2 therapy presented by @kazuki_nozawa. Extremely promising association of ctDNA dynamics with outcomes. Concomitant @ESMO_Open publication! #ESMO26 #ESMOAmbassadors
If extracranial disease is well controlled, this strategy seems more reasonable than the HER2CLIMB-05 approach.
The key question is whether it will remain applicable in the emerging era of frontline T-DXd.
#ESMO26
Primary results from BRIDGET ph2 trial presented by @drsarahsam and published in @ESMO_Open. Adding tucatinib to HP after an isolated intracranial PD led to impressively long PFS (59% free from PD or death at 18mo!!). Reasonable strategy from next Monday in TKI-naive patients.
Bulumtatug fuvedotin (BFv): Nectin-4–targeted ADC with an MMAE payload.
Given the SWITCH trial results with target switching (ORR <10%, PFS <3 mo), payload switching looks promising.
It’s not that simple, but our intuition about ADC resistance is gaining stronger support.
#ESMO26
Switching ADC target does not work. Switching payload does.
Among 60 TOPO1-ADC-pretreated patients with MBC, using a microtubule (MMAE) ADC led to 30% ORR and DoR of 8.8 mo. #ESMO26 #ESMOAmbassadors
Interesting data!
📊One-third of HER2+ eBC patients eligible for NAC had T2N0 disease and were ineligible for DB11. Anthracycline-free THP seems reasonable for this group.
⚖️For T3+ or N1+ disease, DB11 regimen or T(C)HP? Further treatment individualization is needed.
#ESMO26
🔥Impressive results!
But which drives the benefit?
Anbenitamab vs HP?
Or
HB1801 (albumin-bound DTX) vs DTX?
#ESMO26
Presented at #ESMO26:
Among patients with HER2-positive advanced breast cancer, the HER2-targeted bispecific antibody anbenitamab plus albumin-bound docetaxel led to 24-month progression-free survival of 58.5%, as compared with 34.4% with standard therapy. Full phase 3 HEALER trial results: nej.md/4BcWlF3

ALT A forest plot showing subgroup analysis of progression-free survival for patients treated with abemaciclib plus HB1801 versus THP. The left column lists subgroups like age, ECOG performance status, and liver metastasis. The middle columns show patient counts and events. The right column features hazard ratios and confidence intervals.
In DESTINY-Breast04, the T-DXd arm showed a CNS-PFS of 9.7 months and an IC-ORR of 25.0%.
Of course, these are only reference data, as the patient populations are entirely different.

CDK4 inhibitors will likely enter routine breast cancer care.
🧐For 2nd-line endocrine therapy, biomarker-guided treatment based on ESR1, PIK3CA/AKT1/PTEN alterations or CDK4 inhibition?
💡One advantage of CDK4 inhibitors: no CDx required.
#ESMO26
Very promising randomized results with the CDK4 selective inhibitor atirmociclib presented by @antgiorda. Next gen CDKs are here! #ESMO26 #ESMOAmbassadors
Regular MRD monitoring for all high-risk patients is hardly feasible.
We need to better understand the tumor biology associated with MRD positivity and develop strategies to identify patients who truly need MRD surveillance.
#ESMO26
Impressive MRD analysis presented by @YukinoriOzaki. Pts with high-risk beast cancer who, at any point, turned MRD+, had clearly higher risk of recurrence compared to those constantly remaining MRD-. Clinical utility needs to be established prior to broad implementation. #ESMO26
There's a new version of this post
Despite similar pain and emotional functioning with T-DXd and T-DM1, social functioning was worse with T-DXd. Could AEs such as nausea explain this?
If so, proactive, optimal supportive care is absolutely essential.
#ESMO26
PROs from DB05 presented by Carlos Barrios. Preserved QoL with T-DXd (vs T-DM1), with improvement in key domains (pain, arm symptoms) and worsening in social functioning. Overall, they confirm the role of adjuvant T-DXd for patients with high-risk RD. #ESMO26 #ESMOAmbassadors
What remains is the financial burden (perhaps the biggest burden of all?)💰
#ESMO26
PROs from lidERA presented by @GustavoWerutsky. Giredestrant appeared associated with comparable QoL vs standard ET. We don’t every day see a drug that improves adjuvant outcomes without adding side effect burden. #ESMO26 #ESMOAmbassadors
