Just listen to this.
Then consider these polls:
76% of Jewish Israelis think there are no innocents in Gaza (Aug 2025)
82% support expelling Palestinians from Gaza (May 2025)
47% support killing all inhabitants of conquered enemy cities (May 2025)

You funded 10000 new posts for PAs, and funded their degrees, while NOT funding new doctors to be trained as GPs.
You even published on how to dismantle the NHS.
You knew exactly what you were doing.

Referrals to secondary care from GPs used to be treated as they were, requests for further support, investigations, and advice from one experienced colleague to another
They have now, to the detriment of patients, been treated as merely optional
This cannot continue

Referral was meant to be a dance between two clinicians. These days it feels more like a boxing match, with the exhausted GP's guard up, protecting their already battered body .

You have to love the fact that it's been 18 months since this started @NHSE_WTE
And there are still focus groups running on this...
While we await a report which will confirm that the Pope is indeed Catholic.
And lots of things will be said, asked and pontificated over.
But without any funding- thus based on love, vibes and prayers.
Incoming...soon.
Then again, it comes from the stable of those who categorically stood against Motion 5 of the @RCPhysicians EGM passing.
Evans, Powis, McLeod, Dacre, Whitty et al.
Reminder of that motion 5:
"Demand of a formal pause or slow-down in the deployment and recruitment of PAs until wider medico-legal issues, regulation standards, and exact scopes of practice are legally resolved"
History is such a lovely thing.

It was a disastrous mistake in 2004 to scrap family doctors. I tried and failed to bring them back. Will this government do any better?

So , @PatientsAssoc when are you releasing details about how patients can make their voices heard ? When is the deadline ?

The King-Smith family were surprised to find this article in yesterday's @MetroUK
We are delighted that adorable piglet Tom has been rescued, however, whilst we love E.B White's excellent tale Charlotte's Web, Babe was based on Dick King-Smith's The Sheep-Pig. Back to journalism school for you, dear author!

You did NOT get Greenland. Period.
You are the undisputed world champion of lying.
Greenland is NOT yours, never was, and never will be! 🇬🇱
Trump says NATO was "nice enough to give us Greenland. We got Greenland."
Readers added context they thought people might want to know
Greenland is an autonomous territory of the Kingdom of Denmark and is not owned by the United States. A September 2026 security agreement expands US military access but reaffirms Danish sovereignty and Greenlanders' self-determination.

What happened to the #LengReview? A paper exercise to appease the public. Nothings changed, infact this experiment is gaining momentum. @rcgp Doctors demoralised and patients subjected to under qualified substitutes. @YvetteCooperMP this is how you're fiddling the numbers.

Sheffield and St Georges. We should just move all the doctors from there- have all remaining PAs there.
And let all be happy.
Bar perhaps the local population- but then again- when did that matter.

Afternoon lovely people
Politics aside, today is my 52nd birthday and I wanted to ask people from all sides of politics if they’d consider requesting a kit from @DKMS_uk
No cost and takes 5 minutes to complete, it’s free post to return, and you might just save a life 🩸🙏

If we book in to see the Dr to get a prescription and fit note , why are we assigned to a medically unqualified Physician Assistant who can’t prescribe medication nor issue fit notes ?
When will this ridiculous situation end @YvetteCooperMP ?

Ps- i did a 6 year medical course and have practiced medicine for 25 years, passed multiple postgraduate exam and I wouldn't be safe seeing undifferentiated patients in Primary Care. I would be a danger to patients in this setting.
Therefore why do some people have you believe that having no medical degree and doing a two year course somehow gives people magical abilities to be able to see undifferentiated patients in primary care and be safe.
If physician associates are able to train in "medicine" and are magically able to see patients independently in two years - why does it take doctors 5 to 6 years medical degree and 10 years postgraduate training before achieving independence?
We have to decide whether we want a safe healthcare service delivered by fully trained doctors or go back to a free for all where anyone can practice medicine with a cursory level of knowledge/ training and do away with medical degrees / medical training
It's like living in a parallel universe where simultaneously you need over 15 years of training to be considered safe as a doctor but anyone else can do a simple course and get stuck in.

On the day after @peterjukes & I broke the story of Arron Banks’s covert meetings with the Russian ambassador, I was invited onto @BBCr4today to discuss it. Big news! Finally the chance to break through to a BBC audience. Only the interview was cancelled because they needed ‘someone from the other side’ & the Russian ambassador wasn’t available. Facts don’t have a side. It meant the BBC never covered the story. And here we are..

Prediction time- and a bookmark for those who fancy it.
There will be attempts to fudge the PA protocols and dilute by the pro “PA are just differently trained doctors” lobby.
Absolute lock on that Leng will be under heavy pressure to be part of that
Organisations such as Royal Colleges will try to manage it internally and “find a middle ground”
(Colleges will mostly do as asked- unless the uproar is too much to handle)
They will eventually come in the public domain
There will be uproar and much debate as Fellows and Members see them.
The NHS Long Term Workforce Plan will try- again - to shoehorn PAs in.
The basic strategy?
“Let’s find a middle ground”- based on a belief that people are tired of this and will accept.
And a hope that enough has been given to the BMA - pay structures, IMG sacrifice etc for them to sign up
As ever?
The key sits with the supervisors.
Will YOU as a Doctor- agree to taking lesser trained people on and put your liability on the line.
And will you have the time once METR lands.
Policy is one thing - involves much of politics
Implementation?
Needs wide support.
Ask those who tried to force the whole PA project through first time around.
@lengreview @RCPhysicians @rcgp @gmcuk @TheBMA @AoMRC @AoMRCTrainees @rcpsych @JanetEastham
Important update:
Thank you @lengreview for information that relevant clinical protocols have been drafted and sent to stakeholders for opinion
This includes @RCPhysicians and in my view?
Fellows should be able to contribute to that
Have asked @Mumtaz_Patel_1 to consider this is open and not just left to Council etc
@OGMustafa
I am not sure how this information was supposed to be made public without continued enquiries and chasing
Anyway, to next steps.
In addition?
@gmcuk have confirmed that post the protocols being agreed, role of @NHSEngland to ensure jobs stick to that
Failing that? @CareQualityComm role
Let’s get that protocol right and appropriate for PAs working
I don’t give up- till it’s done.
Patient safety stays supreme.
As does transparency.
