The physical exam is a lost art. Telemedicine, AI, automated physicians…. and no one discusses the taking of a great history and performing a detailed physical exam.

I did chest percussion today and the patient told me I was the first doctor that ever did that. Her experience with doctors was surely >100, maybe more. When I was little and sick, my cardiologist Dad always did it. It was always very comforting. When I was in first year, I had trouble distinguishing the “notes” and I asked him for help. He said, “I just feel them on my fingertips”. I presumed he was busting my chops. 25 years later I was making rounds at Tufts with a big entourage. The room was noisy. I percussed a big pleural effusion. A student asked me, “How can you possibly hear the notes?” Without remembering Dad, I reflexively answered, “I feel them in my fingertips”. Physical diagnosis takes a lifetime. Still learning it.

Coronary plaque in younger women may carry a far greater prognostic signal than equivalent plaque in men. In our editorial in the Am Journal of Preventive Cardiology (doi.org/10.1016/j.ajpc.2026.…), we discuss why sex-specific interpretation of CCTA findings is essential—and why even “mild” plaque in women should prompt aggressive prevention efforts. #CardioTwitter #CCTA #WomensHeartHealth

Choosing the “right” procedure and the “right valve” is important for each patient outcome!
I've referred at least 12 Aortic Stenosis patients to our Central Florida surgeons and invariably they opt for surgical AVR in younger patients. Low dose warfarin with an On-X valve seems preferable to a redo at 7-10 years.

More and more, we are finding women’s health experiences are often different than men’s for the same issue and should be studied with this in mind. #healthcareinwomen
It’s very Encouraging that women’s outcomes are more often being looked at separately from their male counterparts.

Repurposing existing devises in Aortic emergencies when time is of the essence and there is no time to order custom materials. We have come a long way in treatment options and knowledge!
🇬🇧 🎯 QUERY: Can a Repurposed Custom-Branched stent-graft save a life in an Aortic Emergency? 💥 When time is the enemy: Adapting custom-made technology for acute aortic emergencies. This Case Report in #EJVESForum details the successful emergency use of a repurposed triple-branched endograft to treat a symptomatic aortic arch aneurysm. 💡KEY FINDINGS: · The Challenge: An 84-year-old male presented with acute chest pain and a rapidly expanding 6.5 cm arch aneurysm following a type B aortic dissection. · The Dilemma: Total endovascular arch repair typically requires custom-made multibranched devices with a 6–12 week manufacturing delay—time this patient did not have. · The Innovation: A triple-branched endograft, originally custom-manufactured for another patient’s anatomy, was identified as a suitable fit. The procedure involved a total endovascular approach, catheterising the innominate, left common carotid, and left subclavian arteries through the device’s internal branches. · Outcome: Technical success was achieved with exclusion of the aneurysm and preservation of all supra-aortic branches. The patient was discharged on day 7 with no neurological complications. 🚀 TAKEAWAY: Resourcefulness in the Emergency Setting Custom-made multibranched endografts are the gold standard for elective arch repair but are limited by production lead times. This case highlights that "off-the-shelf" repurposing of available custom devices is a viable life-saving strategy in emergencies. Precise anatomical matching using 3D workstations remains mandatory to ensure internal branch compatibility and successful supra-aortic revascularisation. We would like to see more studies directed towards #womendissection survivors. Much of the research has concentrated on men, with the exception of AD during pregnancy or childbirth. Eg. Women may experience different symptoms and recovery issues.
