Hear ye hear ye!! New pembrolizumab TMB tumor-agnostic threshold is 13 rather than 10. 🔥
Our data suggests it is 16 mut/mb (best threshold)
Data coming soon!
Responses can be phenomenal. I have patients who went from “next step hospice” to “CR ongoing for years— aka cured”

Can’t believe we’re having a full session on KRAS G12D targeted therapy in pancreatic cancer! So glad to see how far we’ve come.
@DrShubhamPant @DAVAOnc @BaptistHealthSF @BHAcademics @MiamiCancerInst
Dr. @Sarbaji85064063 of @BaptistHealthSF discussed setidegrasib, a KRAS G12D-targeted degrader in pancreatic cancer, highlighting its mechanism, phase 1 efficacy and safety data, and ongoing phase 3 evaluation in first-line metastatic PDAC. #DAVAGI
I had a chance to read Matt‘s book and truly it hit home on so many levels. Congratulations @MatthewZachary I hope more people get a chance to read your book and the message resonates with them as it did with me.
HOLY. FUCKING. SHIT.
That is We the Patients in the front window of the flagship Barnes & Noble on Fifth Avenue in Manhattan.
Not tucked into Health & Wellness. Not spine-out on a shelf somewhere. THE WINDOW.
And then I walked inside and found another pile sitting on the Must-Read Nonfiction table.
I wrote We the Patients: Understanding, Navigating, and Surviving America’s Healthcare Nightmare after 30 years as a patient and 25 years professionally yelling about what this system does to actual human beings.
Somewhere between writing the damn thing, Wiley publishing it, the second printing, the third printing, the reviews, the emails from strangers, and watching this weird little patient uprising start finding its people, I apparently forgot that occasionally you get to stop and look around.
Today I looked around.
Fifth Avenue. Barnes & Noble. Front fucking window.
There are a lot of people who helped get this book from an idea rattling around in my head to that. And there are a lot of patients whose stories, anger, absurdities, heartbreak, and survival are inside those pages.
So I’m going to do something I’m historically terrible at and just take the fucking win.
Holy shit.

A thoughtful piece from @MatthewZachary about the journey of patient advocacy and where it’s headed and rather where it needs to go.

I Was In a Rut
I was in a rut, worn thin by the day,
frustration piled up in the ordinary way—
patients and paperwork, a job never done,
tired in my bones before the day was begun.

I was in a rut. I remember who I am,
I remember those I love, and those who give a damn,
the ones who call my name, those who shout—
I was in a rut, but this is my way out.

Replying to @NCCN
@NCCN please rapidly update the #pancreaticcancer guidelines.
Insurance company AI denying patients Daraxonrasib and recommending I try gem/erlotinib first. Probably because it remains on guidelines for a 2 week survival advantage! I hope the guidelines will consider removing

Kudos to @IowaMed MS4 Matthew Gao for leading this work describing outcomes in early-onset versus average-onset small bowel NETs using the SEER registry. More than a few studies (albeit all retrospective) now showing better outcomes in early-onset NETs (potentially better biology?), but a lot remains unknown still. This is an active area of research, work in progress.
Matt is applying for Internal Medicine residency this year and any program will be lucky to recruit a thoughtful, smart and driven future physician like him. @RPatel20166 @jason_semprini @michael_ororke @SethConcorsMD @ChandrikhaC @WinshipAtEmory @TheGutOncLab

Pleased to announce as of next week that I will be joining @NateraOncology as a medical director in GI oncology.
I’m grateful for the 8 years at UTSW and the opportunity to grow as a GI oncologist. Looking forward to advancing the science of MRD and precision onc in GI cancers

Expert consensus statement on the use of incretin mimetics in patients with NENs, eloquently summarized by @OncoThor in the tweet below. Grateful to be a part of this effort. This statement not only provides guidance/expert advice on tackling this challenge in clinic when seeing patients but also highlights the need for robust prospective evidence to guide future use of these agents. @NANETS1 @ENETS_ORG @WinshipAtEmory

A new Clinical Practice article summarizes the classification, evaluation, risk stratification, and management of syncope. History, examination, electrocardiography, and orthostatic blood-pressure measurement identify the cause in most patients.
