This is how Monty Python got started.
I’m a doctor. Here’s why childhood vaccination for hepatitis A is important.

For a while I have voiced that all PCPs should terminate Medicare and insurance contracts. We continue to whine when Medicare cuts happen. We celebrate a 0.3% increase when hospital systems and MA plans get big increases. If you are still in the system, blame yourselves.

Healthcare’s top lobbyists just dropped nearly $285 million in the first half of 2026 alone. Meanwhile #DirectCare spent exactly $0. We don’t buy influence. We just take care of patients and let the receipts speak. beckershospitalreview.com/fi…

Opt out. If not now, have an exit strategy before 2027.
OPT. OUT. NOW. “There is also a perverse incentive CMS has not addressed: if this is finalized, the rational response for physicians is to split these encounters across two visits. The patient returns on a separate day for the procedure, each encounter is billed at 100%, and Medicare pays more in aggregate while the patient is significantly inconvenienced. Has CMS modeled the economics of this very obvious response from clinicians?”

Y'all If you have not terminated Blue Cross or Cigna or whoever as your " insurer" and joined a HealthShare by now, what are you waiting for??? Really.

If you are a primary care doctor in the insurance model, or worse yet, employed, develop an exit strategy. Rip up insurance and Medicare contracts. Create or join a direct primary care practice. Help is available.

Healthcare has confused financing with insurance. Routine care should be purchased. Catastrophic care should be insured. Most employers have never been shown the difference.

All primary care docs belong in our DPC world. Off the grid. Paradigm shift.
The IRS just raised HSA limits by $100. In the insurance world, $100 is: one copay and a parking ticket. In direct primary care, $100 is: - 27 YEARS of blood pressure meds ($0.01/pill, wholesale) - A complete executive-physical lab panel — CBC, metabolic panel, liver, thyroid, A1c, lipids, PSA, urinalysis, B12, iron, and more ...for you AND your spouse. 16 tests each. With change left over. Same dollar. Different system. #DirectCare

The @aafp needs to go full-bore to help us leave this "practice environment." All FPs need to terminate insurance involvement. No more groveling asking for crumbs. That does not work. It never will. Terminate all insurance contracts and Medicare/Medicaid affiliation as of 1/1/2027. Thank you @aafp for supporting DPC. It is time to go all-in.
This and similar events around the country should grab our attention and embarrass those who have created this untenable practice environment. They are leaving because the complexity of the insurance-based system has broken their will to keep their practice open.

DOJ should now indict HHS & Congress for overpaying hospital services vs independents, a fraud that dwarfs this piddling amount and one that has lined DC pockets for decades. This theater is a decoy/distraction from Uncle Sam, the ultimate fraudster. GKS
The DOJ just announced the National Health Care Fraud Takedown – 455 defendants charged, including 90 doctors and licensed medical professionals, in connection with over $6.5 billion in alleged fraud.   Thank you to @TheJusticeDept and the @WHFraudTF for this historic effort to protect American taxpayers and patients.

Leadership in elected office means servant leadership. It means serving the people, standing on your record, and answering tough questions. I’ve spent my entire adult life serving others—in the military, in medicine, in business, and in public office. Meanwhile, after five years in Congress, Julia Letlow has not had a bill signed into law and, in these final days of the campaign, she still refuses to debate the issues facing Louisiana. Instead of making her case to voters, she’s spending millions on attack ads, filing lawsuits, and claiming she’s being treated unfairly. If a candidate won’t show leadership while asking for your vote, how will they lead in the U.S. Senate? Watch. – at Louisiana, USA

Thriving is only possible outside of the machine. A PCP can make money in the system but at a heavy non-monetary cost.
You do NOT need the hospital. You do not need the insurance contract. You do not need permission. The biggest lie in medicine is that you can't survive outside the machine. I'm proof you can thrive.

This is completely true.
Imagine if every passenger on an airplane had a direct line to the pilot. That’s what medicine has become. We have embraced frictionless communication and that not a good thing. With things like Epic Chat, every member of the healthcare team has immediate access to the physician. Now there’s a constant stream of minor questions. It’s discouraged people from thinking critically. “Just ask the doctor,” has replaced any sort of clinical reasoning. When I was a resident, we had an answering service. Anyone who wanted to reach the doctor needed to go through a third party. This provided triage and accountability. If we were getting called in the middle of the night for stool softener orders, there was a record of that. A little friction in communication is a good thing.

In the upcoming Louisiana Senate election we have the chance to put a conservative America First in office. I would hope that @fleming4senate would consider looking at 2025's S. 3386 and H.R . 6703 as a starter point in health finance reform.

I hope this means studying HCQ and IVM
Today, I signed a targeted PREP Act declaration to support the development and deployment of medical countermeasures related to Andes virus, which can cause the deadly respiratory illness Hantavirus Pulmonary Syndrome. This action helps remove barriers to research and response efforts while we continue monitoring the recent outbreak linked to the South Atlantic cruise ship. HHS is taking this situation seriously and will continue working to protect public health and support the safe development of potential treatments and countermeasures.

The @aafp recommends that at 21 year old at 30 weeks gestation get a COVID vaccine. The largest FP organization is dead serious about that. Malpractice.

In the April 2026 @aafp Journal, Dr. Coles published an article "Vaccine Adverse Effects: An Overview." It was lame but in keeping with the Pharma narrative. It was actually embarrassing as it starts "one of the most successful medical advances..."
