Tremendous discussion by @drsarahsam, highlighting the promise of several escalation/substitution strategies across breast cancer subtypes, but reminding the costs and caveat of each new option. #ASCO26

Dato-DXd now approved for the 1L treatment of IO-ineligible mTNBC.
For most pts, both Dato-DXd & SG will be adequate choices.
For some, based on the data, Dato-DXd may be preferred:
-recurrence <6 mo
-symptomatic / response needed
-brain mets
-living far from hospital (Q3W)
A good read for those in medicine and leaders alike (or both).

Powerful editorial on corporitization of medicine.
1. Trainees are not cheap labor
2. Margin focus threatens trust.
3. Leaders need to stay in touch with clinical care

My favorite study today at #ESMOBreast: PREcoopERA (IBCSG 67-22), a window-of-opportunity trial in premenopausal ER+/HER2- early BC asking whether giredestrant alone is enough, or whether ovarian suppression is still required.
231 patients → giredestrant alone, G + triptorelin, or anastrozole + triptorelin. Primary endpoint: Ki67 at 4 weeks.
Giredestrant alone did NOT meet non-inferiority vs G+OS (-68% vs -80%). And G+OS was not superior to anastrozole+OS (p=0.18).
Bottom line: Even for a next-gen SERD, ovarian suppression adds to anti proliferation effect in premenopausal patients.

In rural health care, the path to a timely breast cancer diagnosis and treatment is often blocked by an accumulation of obstacles, according to MinhTri Nguyen, MD.

Ovarian function suppression is arguably the most important advancement for ER+ young women with breast cancer, markedly reducing recurrence.
Now more trials on how to support side effects for these women thrown into early menopause. #bcsm

Trial design insufficient to answer the clinical question (Not that Cami isn't effective -PFS hazard ratio of 0.44 is real). The question: whether ctDNA-guided early switching is better than switching at radiographic progression. Awaiting the PDUFA date and FDA final say.

There will be people who agree & disagree with @US_FDA decision on #SERENA6 #cami, but fantastic document by FDA 👏 educating & clearly outlining reasons for the decision and how PFS2 will be interpreted.
A must read for drug developers and pharma.

Very important data. Even in the absence of oral SERD at crossover, there is no apparent OS benefit as yet for switch based on molecular ESR1mut detection without radiological progression. And in current practice, all patients would get an oral SERD at radiological/clinical PD

A subset of patients with HER2+ metastatic breast cancer are cured by current therapies. Can we increase the rate of cured patients with patient selection & sequential use of the best anti-HER2 agents currently available? High hopes in the #SAPPHO phase 2 trial to bring answers.
The new #SAPPHO study, led by @DFCI_BreastOnc & @TheTBCRC, is evaluating a novel treatment approach for HER2+ #MetastaticBreastCancer. The study will help investigators understand if giving sequential treatments is safe and effective for patients with newly diagnosed HER2+ #MBC, and if maintenance treatment can be safely interrupted.
