Living and Practicing By Design™ - Not Default dpcnews.com/practice-tip/liv… I’m down here in Fort Lauderdale today, and I just spent the entire day with some incredible businesspeople. Literally, I am learning from some of the smartest and biggest businesspeople in the world. Millionaires and billionaires. There are three things I want to tell you that are takeaways from things I’ve known for years, but they were just driven home today. Number one is you’ve got to design your life first. What do you want your life to look like? What do you want to do with your recreational time, your downtime, et cetera? That’s huge. You see, when we feel like doing this lacking, a real purpose behind what we do, life and practice becomes so much more difficult. And it doesn’t have to be that challenging. My biggest motivation has always been and will continue to be personal and financial freedom. Number two Is to have longevity In your practice, you need to simplify everything. Healthcare especially can get absolutely crazy. Especially today. Unless you have a proper design to make you the unique physician for your community you’ll always struggle. And this changes over time especially if you want to continue to grow and expand and serve more people which is the reason I came here for three days! Number three, rather than delegating all tasks to everybody else, which ultimately, you may end up doing a large portion of, you’ve got to know what the key steps, components and operational systems are. For example, one of the biggest mistakes that oftentimes gets made with practice marketing is that you hire a marketer. What happens? You don’t even know the basics. You have no idea what they’re doing. You spend an absolute fortune and have little or nothing to show for it. Because you can’t even ask them the right questions. Next, apply this strategy to almost everything else that it takes to run a private practice successfully. Guys, listen up. It doesn’t have to be that hard. I first started to talk about this in 2007 when I published Living and Practicing by Design, Saving the Hearts that Care for Our Lives, which I’m currently rewriting and updating. The takeaway is this. Spend time learning these, even just a little bit every day, and dive in deeper when you need. You’ll ultimately have a lot less stress, much better income, plus the ultimate freedom and joy that come from running your own show. Remember, this is a choice, too

You weren’t always just a doctor. Before the pager. Before the notes. Before the relentless schedule, the night shifts, the checkboxes, and the protocols—there was you. A whole person, full of interests, curiosity, humor, and vision. But in the process of becoming a physician, too many parts of you were set aside. Pushed down. Delayed until “someday.” And for many physicians, that “someday” never comes. Unless you decide it does. What Gets Lost in the Process Medical training teaches focus, sacrifice, and resilience—but it rarely teaches balance. And so, over time, you may have lost: •The hobby that used to bring you joy
•The friendships that once grounded you
•The spontaneous laughter that used to come easily
•The sense of identity outside your clinical role
You became efficient, reliable, respected—but maybe also tired, disconnected, and unsure who you are when the white coat comes off. You Can Reclaim What You Put on Hold Here’s the good news: those parts of you aren’t gone. They’ve just been quiet. Waiting. Reclaiming them doesn’t mean giving up medicine—it means living it from a fuller, more human place. Start by asking: •What did I used to love before this pace became normal?
•What kind of person do I want to be outside of work?
•What would it look like to prioritize even one small part of that?
You don’t need a full reinvention. You need a reconnection. “You are allowed to be more than your productivity. You are allowed to be whole.” Reflection Prompt What’s one part of you that’s been on hold—and what’s one thing you can do to bring it back this week? Ready to Rediscover Who You Are—Outside the Exam Room? If you’re craving more clarity, energy, and identity beyond your professional title, now’s the time to start. perfectpracticeweb.com/post/…

Independent oncologist: pays full price for the drug. Same oncologist, the day after a hospital buys the practice: 50-60% off. Same doctor. Same patients. Same drug. That's how 340B turned into a consolidation machine.

Direct care and direct insurance are the future of outpatient medicine as the health insurance bubble continues to burst. Fixing health insurance premiums will help restore the middle class.

Independent Physicians Sleep Better—Here’s Why It’s 2 a.m. You’re wide awake again. Running through tomorrow’s schedule. Worrying about unfinished charts. Dreading morning clinic. If this sounds familiar, you’re not alone. Chronic sleep disruption is common among physicians especially those stuck in high-pressure, system-driven models of care. But here's the good news: physicians in independent practice report significantly improved sleep quality, energy, and emotional recovery. Why Employed Doctors Can’t Rest In traditional settings, poor sleep often stems from: Long clinic days with back-to-back patients After-hours charting on EMRs not designed by physicians On-call demands that disrupt circadian rhythms High stress loads from lack of control over time and decisions The result? You leave work, but your mind doesn’t. Independence = Better Rest Physicians who run their own practices, especially in Direct Primary Care, concierge, or micro-practice models, experience a dramatic shift: No late-night charting after 30 patient encounters Control over their schedule, allowing time for rest and recovery Less administrative tension, leading to calmer nervous systems Fewer sleep interruptions, as many opt out of hospital call They’re not just sleeping more—they’re sleeping better. “I didn’t realize how much the system was costing me until I got my first full night’s sleep in years.”— Dr. M., Independent Internist Why Better Sleep Matters As a physician, quality sleep supports: Clinical judgment and focus Emotional regulation Hormonal balance and cardiovascular health Long-term cognitive performance It’s not a side benefit, it’s a medical necessity for your well-being and your patients. Want to Sleep Like a Human Again? If poor sleep has become your normal, it’s time to question the system not your resilience. Let’s explore how independent practice can restore rest and revive your quality of life. perfectpracticeweb.com/post/…

If every doctor understood how this feels… To practice medicine freely… Under your own roof with your own patients and how much you care for them when you’re not part of the system that poisons everything… The system would likely collapse and need to be rebuilt. By physicians and patients. I think this has to happen anyway. 🥲

Super important to understand about the ER 👇💯
"You can't have a free market in healthcare — what about emergencies?" 95% of ER visits can be shopped. Only 5% of patients truly cannot wait. We don't need an entire socialized system because of 5%. — Dr. Anthony DiGiorgio

The same MRI is $300 in one building and $4,000 across the street. Same machine. Same images. Same radiologist. The only thing that changed: a hospital bought the building and tacked on a "facility fee." You're paying rent on a room you sat in for nine minutes. Ask for the cash price before you ever schedule one. FYI: This is the 9th time in 9 years that I have posted this exact message. 😊
