Direct care and direct insurance are the future of outpatient medicine as the health insurance bubble continues to burst.
Fixing health insurance premiums will help restore the middle class.
As health care grows more complicated and insurance prices continue to rise, some patients are turning to a different model for routine medical care: direct primary care.

If every doctor understood how this feels… To practice medicine freely… Under your own roof with your own patients and how much you care for them when you’re not part of the system that poisons everything…
The system would likely collapse and need to be rebuilt. By physicians and patients.
I think this has to happen anyway. 🥲

The same MRI is $300 in one building and $4,000 across the street.
Same machine.
Same images.
Same radiologist.
The only thing that changed: a hospital bought the building and tacked on a "facility fee."
You're paying rent on a room you sat in for nine minutes.
Ask for the cash price before you ever schedule one.
FYI: This is the 9th time in 9 years that I have posted this exact message.
😊
Great to see that #DirectCare is being used by gubernatorial candidates and discussed as solutions for states to make healthcare affordable again.

Most people think you need pills or probiotics to fix your microbiome…
Recent research showed that daily almond snacking significantly boosted Faecalibacterium prausnitzii, a key beneficial bacteria.
Chomping on almonds also bumps up butyrate, a short-chain fatty acid that protects and repairs your gut lining. 🥜
One of the easiest, most evidence-backed daily swaps you can make. No fancy protocol required.

People are touting ivermectin for Hantavirus. A drug that requires a supply chain, a seller, a buyer and money. If IVM actually worked, it would immediately be hoarded and not be available. Nevermind that there are no trials showing that it works.
On the other hand we have trials that interferon (an endogenous protein in your body) helps fight all sorts of viruses and we have trials that it helps in Hantavirus.
We also know cheap ways of elevating our own interferon quite high. So why are some against it? Because you don't need a supply chain, pharmacist, or money to get it. You can't hoard it either.

Should employer health plans stop using insurance for routine care? benefitspro.com/2026/05/01/s… Yup because #DirectCare can make most care too cheap to insure.

This collab with Weimar University is the most popular podcast ever on their channel (youtube.com/@WeimarUni). We discuss sunlight and NEWSTART at the actual University that coined the term and has a program called NEWSTART!

I have watched patients in their seventies start from positions that would make a physical therapist flinch, and rebuild themselves into versions of themselves their own families barely recognized.
Not by becoming athletes. By doing small, specific work, consistently, for months and then years.

🚨I think there are a lot of healthcare providers/skeptics that are ignorant of where we are right now with respect to sunlight and infrared light in medicine.
Here are the statements that I'll make with representative peer-reviewed publications: (feel free to use this a an information packet to send to those that are interested).
1) Sunlight contains infrared light which has been shown to penetrate all throughout and completely through the human body (and clothes) and causes immediate and real physiological changes that are measurable.

2) Trees and grass are highly reflective of this type of light:
unh.edu/unhtoday/2021/04/see…
3) Makes COVID-19 patients in hospital discharge 30% faster in an RCT:
pubmed.ncbi.nlm.nih.gov/3649…
4) Makes ICU patients discharge 30% faster and also stronger in an RCT:
pubmed.ncbi.nlm.nih.gov/3826…
5) Improved glucose control in healthy individuals in a RCT:
pubmed.ncbi.nlm.nih.gov/3837…
6) Prevented pseudomonas bacteria from producing biofilm - a major problem in fighting bacteria.
nature.com/articles/s41467-0…
7) You don't need for it to hit all over your body because the mitochondria talk to each other in ways that we don't fully understand. pmc.ncbi.nlm.nih.gov/article…
8) Those exposed to this type of light for a lifetime live longer
pubmed.ncbi.nlm.nih.gov/2699… and pubmed.ncbi.nlm.nih.gov/3909…
9) The LED lighting environment is unhealthy given that it doesn't have infrared light. Adding infrared light to that environment dramatically improved vision as a measure of broader mitochondrial activity in a study:

Tesla built an assembly line BY HAND in a parking lot tent before automating.
Healthcare keeps "digitizing" broken processes. EMRs automated the paperwork without asking if it should exist.
DPC strips it back: doctor + patient + relationship. Then build.

After researching the effects of infrared light on mitochondrial health and understanding that mitochondrial dysfunction is the cornerstone of long covid I started recommending intentional sun exposure to my patients.
When I described this some people said that my recommendations were dismissive and that I didn’t truly empathize with long covid sufferers
To the contrary:

The is one of the most prestigious scientific journals in the world now acknowledging the benefits of long wavelength light (red or infrared light) therapy and the fact that we are indoors more now than ever before.
For those that have had to endure scorn from fellow clinicians when talking about the benefits, this is validation. For those who still think this is woo woo, open a journal and your mind and realize that the world is passing you by. This is now the science!
A growing body of legitimate science has been exploring the benefits of red light therapy for several conditions, from ADHD, to retinal degeneration, to dermatology

“A hospital in Barcelona sits right across from the beach. So they started wheeling ICU patients to the shore. Ten minutes of sunlight and waves — and doctors say it changes everything.
Hospital del Mar in Barcelona sits directly across from the Mediterranean. For years, staff have been wheeling ICU patients to the beachfront as part of a program to "humanize" intensive care. Doctors say even 10 minutes of sunlight and waves can improve a patient's well-being and recovery.
The program gained worldwide attention during COVID-19, when patients who spent weeks sedated on ventilators were taken to the beach. One patient, after 52 days in the ICU, said: "It's one of the best days I remember." Staff monitor vital signs throughout each visit.
Dr. Judith Marín said the team had been doing therapeutic outings for two years before the pandemic. The hospital is now studying whether beach visits improve long-term recovery.”
