Well well well…
This was certainly my favorite RCC / kidney cancer presentation and heralding a significant shift in the RCC landscape!
45% response in heavily pretreated pts. Highly impressive!
Needs to move into frontline combinations! Congrats @montypal + colleagues

Exciting day in GCT as @DrDarrenFeldman presents data on ixo-V, a CLDN6 ADC. High response rate in refractory setting. More patients will be needed to evaluate durability and responses across histologies

🚨 #ESMO26 | #Podium2Screen — LITESPARK-012
@OncoAlert
A useful reminder in advanced #RCC:
Adding more active drugs does not necessarily create a better first-line regimen.
@DrChoueiri presents phase III LITESPARK-012, evaluating whether adding either belzutifan or the CTLA-4 antibody quavonlimab to pembrolizumab + lenvatinib could improve outcomes.
📊 n=1,688
➡️ No improvement in PFS
➡️ No improvement in OS
➡️ No improvement in ORR
➡️ Higher treatment discontinuation in the triplet arms
🎯 Why this matters: negative trials can be highly informative.
They tell us that simply layering another mechanistically attractive agent onto an effective backbone is not enough. Biology, sequencing, toxicity and patient selection still matter.
For me, the message is clear:
pembrolizumab + lenvatinib remains an established 1L standard — and future intensification needs a stronger biological rationale, not just another drug.

HARMONi-GI1, presidential session at #ESMO26.
@UGrewalMD guessed .7 HR
I guessed .8 HR
We were equally right (.75 HR).
Phase 3, 1L advanced biliary tract cancer: ivonescimab (PD-1 x VEGF) + GemCis vs durvalumab + GemCis. 682 patients, median follow-up 14.3 mo.
OS 14.9 vs 12.1 mo, HR 0.75 (95% CI 0.61-0.92), p=0.0031
PFS 8.4 vs 5.8 mo, HR 0.61 (95% CI 0.51-0.72)
ORR 53.8% vs 32.5%
This was an appropriate comparator for 2026 and compelling data (although really did need every one of those patients to power).
Probable new first line once the full data comes out.
The bispecifics era is in full force.

The FDA has converted the accelerated approval of pembrolizumab (IV) or pembrolizumab and berahyaluronidase alfa-pmph (SC) to traditional approval for patients with previously treated unresectable or metastatic TMB-high solid tumors, with an updated threshold of ≥13 mutations/Mb.
Based on the phase 2 KEYNOTE-158 trial, this approval highlights the evolving role of TMB as a predictive biomarker for immunotherapy and the importance of refining biomarker thresholds based on clinical efficacy data.
Another important step in the convergence of precision medicine and immunotherapy.

Proud to be part of @OncoAlert . With the wealth of data coming out of #ESMO26 its helpful to have some select studies to key in on. Instead of taking one persons list we put together a group list based on Delphi consensus rules.
Immunotherapy and vaccine therapies clearly stealing the show, although still room for next generation targeted therapies too!
