Guideline developers often don’t clearly understand the concept of indirect evidence. Study that evaluated guideline recommendations labeled as expert opinion found most of these recommendations were in fact based on indirect evidence.

Someday I hope that trialists will realize that the purpose of a parallel group randomized trial is to compare parallel groups not to compare change from baseline. Baseline should only be used as an adjustment covariate (hence not falsely assuming a slope of 1.0 of pre vs. post)
