1/ MYOCARDIAL BRIDGES (MBs) ANOCA mechanisms: - Slow early diast relaxation - Diast vessel restriction +/- neg remodeling - Branch steal (Venturi effect) - Proximal atherosclerosis - Associated vasospasm, CMD (common!) (see next for MINOCA)

1/ Step-by-step “CORONARY FUNCTION TESTING” for #ANOCA #INOCA PREPARATION: 1. Hold all vasodilators for 48 hours (exceptions = hydralazine, clonidine, doxazosin) 2. Hold caffeine for 24 hours (for adenosine) #CardioTwitter

Nice case @RSohnMD @jl35wilsonMD and agree similarities to the Acute Mechanocardiac Coronary Artery Disruption hypothesis #AMCAD nitter.gq/rajivxgulati/status/17…
Yes, as relates to stress-induced CM. Seems like MB is same concept — like this case and the other one I posted recently 👇🏼 — not so “spontaneous”

1/ Thanks @jdrwilcox. Yes, more comfortable doing this as a CTO IC. Here's my approach: 1. Anticipate #SCAD before cath 2. Avoid aggressive catheter engagement 3. Gentle injections w/ limited views 4. Leave nearly all lesions alone 5. When to PCI? See next ... #CardioTwitter

An OCT conundrum during PCI - how did one coronary lesion become three lesions by OCT? 🤔 View this case presented by @RSohnMD - originally shared on Twitter & selected by @Ortega_Paz for publication on PCRonline. ow.ly/gsUm30sffYM #cardiotwitter #cardioed

To avoid a climate disaster, we need to eliminate emissions from the ways we create electricity, grow food, make things, move around, and heat and cool our buildings. It won’t be easy, but I believe we can do it. This book is about what it will take. gatesnot.es/2ZdYWO2

Very nice algorithm for management of SCAD using CTO skills and imaging first techniques!

Interesting case for friday afternoon. My last case of 2019. LAD/Dg bifurcation with very difficult SB access. “Streamline Reverse wire technique” followed by IVUS guided mini-crush. Hoping the best for all my #cardiotwitter friends.

What is the heart rate of the largest animal in the world–-the blue whale? They measured it for the 1st time! With diving, 4-8 beats per minute (as low as 2). That's incredible. pnas.org/content/early/2019/… @PNASNews by @GoldbogenLab @Stanford w/ @Scripps_Ocean's Paul Ponganis et al
