The pulmonary valve. 👀
With 3D echo, we can appreciate even the thin, normal PV in short axis—almost impossible to obtain with 2D.

Subaortic VSD with an Unruptured Right Sinus of Valsalva Aneurysm in 23 y.o patient.
• Subaortic VSD partially overridden by the aortic root
• Rightward displacement of the aortic root
• Elongated, unruptured aneurysm arising from the right coronary sinus
It seems to me that this aortic insufficiency is the result of a diseased valve with several cracks. What do you think?

LV apex mechanics at different stages of apical HCM: as apical rotation⬇️ assoc with NYHA class ≥ Ⅱ & clinical events
🫀pure: LVH only in apex below pap muscle:⬇️apical rotation & global twist
🫀mixed: LVH extends to mid-ventricle:⬇️mid-ventricular strain & ⬇️global twist
🫀relative: early stage abnl apical strain but rotation nl

⬇️apical rotation leads to ⬇️early LV diastolic suction which is needed for efficient LV filling
rotation counterclockwise systolic rotation of LV apex in SAX 🆚 twist/wringing base-to-apex gradient in rotation angle along longitudinal LV axis
