Proud to report at #ESMO2026 the results of PE-PE trial. Oligometastatic #RCC pts have a significant benefit when pembro was added to MDT irrespectively of the type of MDT used and up to 5y from previous surgery.
Thanks to the 🇮🇹 team and special thanks to @DrChoueiri!

MASTERPLAN full publication is out!!
The RT volumes are worth a closer look. CTV = primary GTV + nodal GTV + tumour-vessel interface. The TVI extends 5 mm proximally and 5 mm distally along involved/closely adjacent vessels, for patients where surgery is likely. PTV = CTV/ITV + 5 mm. No elective nodal irradiation, and no elective perineural or celiac/SMA plexus volume specified in the protocol. That is a focused target, with gross nodal disease included.
Real-time RTQA is a big part of this story. Review identified 36 major planning deviations in 12 patients, reduced to 2 deviations in 2 patients after QA. All plans passed independent review before treatment. As a radiation oncologist, I want that detail alongside the outcomes. Reproducing the treatment means reproducing the contour review, OAR constraints and image guidance too.
Also worth keeping the treatment delivery denominator in view: only 40/49 patients assigned to SBRT actually received it (82%). The efficacy analysis is intention-to-treat. It should not be read as an outcome series of patients who all completed SBRT.
The surgical numbers need some unpacking. All comparisons below are chemo + SBRT vs chemo alone:
✅ Underwent surgery: 62% vs 44% (29/47 vs 11/25).
✅ Completed resection: 53% vs 44% (25/47 vs 11/25).
✅ R0 among all resected patients: 52% vs 36% (13/25 vs 4/11).
✅ R0 in the resected POPC subset: 62% vs 40% (13/21 vs 4/10).
POPC is the potentially operable cohort. So the 62% surgery figure includes exploration without resection, while the 62% R0 figure applies only to resected POPC patients. These are different endpoints with different denominators. None of those between-arm surgical comparisons was statistically significant. The paper also defines R0 as more than 1 mm clearance.
Adjuvant chemo is another important difference: 80% vs 36% of resected patients received it (20/25 vs 4/11). That complicates attribution within the treatment pathway. Early progression in the control group may have contributed, so this is a post-randomisation imbalance with more than one possible explanation. The post-hoc analyses were directionally consistent, but the authors explicitly caution about selection bias.
For the planning details: PTV_EVAL excluded the GI PRVs, with a minimum 3 mm PRV expansion. OAR constraints took priority over target coverage. Duodenum, stomach and small bowel D0.5cc were each constrained to <33 Gy, with V30 <5 cc. Delivery used standard linacs; breath hold was recommended, and fiducials were placed in 35/40 patients (88%). This is relevant to how we would implement the approach in practice.
With no elective nodes, isolated regional nodal relapse was 4% vs 8% (2/49 vs 2/25). Reassuring, although that does not establish the optimal elective volume (especially considering these patients went to surgery - ASTRO/NRG recommends inclusion of elective perineural coverage). Overall G3+ AEs were 70% vs 64% (33/47 vs 16/25). One G3 pancreatitis and one G2 small bowel obstruction were attributed to SBRT. The practical takeaway for me is to keep the target definition and prospective QA attached to the results, and to be precise about what the surgery and R0 numbers actually measure.

🚀Upper GI oral abstracts #ESMO26 Day 1 summary results
1) Arcotatug tavatecan improved PFS in CLDN18.2+ G/GEJA in the 3L+ setting
2) SANO trial, OS at 5 years similar between active surveillance and surgery in patients with oesophageal cancer and a complete clinical response after nCR, despite more distant metastasis
3) Adding pembrolizumab to dCRT did not improve EFS in untreated unresectable esophageal or GEJ cancer with PD-L1 CPS ≥10
4) Daraxonrasib demonstrated consistent OS benefit in different RAS mutatin subgroups in RASolute-302
5) mKRAS-specific lymph node targeted amphiphile immunotherapy in PDAC did not met primary DFS endpoint.
6) Zani + CT +/- tisle keep OS benefit in at IA2 of HERIZON-GEA-01
7) Anbenitamab plus ChT improved OS in patients with HER2-positive GC/GEJC adenoca after previous trastuzumab
8) TQB2102 (HER2 bispecific antibody-drug conjugate) plus bemotrizumab and capecitabine demonstrates promising antitumor activity in 1L of advanced GC/GEJC
9) Dostarlimab in Pts with localized dMMR/MSI-H GEA led to a 62.5% 1y-cCR at the first interim analysis, enabling organ preservation.
10) FOLFIRI and Ram vs paclitaxel in taxan-pretreated GEA imporved PFS but no OS
11) Stage II–IIIB gastric cancer after D2 resection, four months of adjuvant XELOX was noninferior to six months for 3-year DFS
12) Adding durvalumab to dCRT in locally advanced esophagus carcinoma did not improve PFS
13) Adding SBRT to MFOLFIRINOX in high risk, borderline resectable or locally advanced pancreatic cancer mproved LRR and PFS
14) FGFR2b expressing GC/GEJ with co-existing FGFR2 amp may define a biologically distinct subset with potential sensitivity to bemarituzumab

🚨 ESMO 2026: A new treatment algorithm for localized gastric & oesophageal cancer!
🔹 Oesophageal SCC: CROSS → Surgery → Adjuvant nivolumab (if residual disease)
🔹 Gastric/GEJ adenocarcinoma: Perioperative D-FLOT + durvalumab maintenance is now preferred.
🔹 Early-stage disease: Endoscopic or surgical resection.
🔹 Unresectable/unfit: Definitive chemoradiotherapy.
The key shift: Immunotherapy is now integrated into curative-intent treatment for selected patients.

Every cancer organ gets the same sequence: surgery is the standard, then trials show you often don't need it, then surgery becomes the salvage.
Breast, larynx, limb, rectum, esophagus, mesothelioma.
Bladder is next. Patients want their organs. 🧵

🇯🇵 Provocative update from JCOG 0802 again shows that lobectomy is associated with inferior OS when stage I NSCLC is managed surgically. This time, among pts with high-risk pathological findings (LVI, VSI, pleural invasion, or LN metastasis). This rekindles the aphorism that wider surgical margins don't always = better outcomes, even though lobectomy improves RFS. jto.org/article/S1556-0864(2…

Post-prostatectomy RT in just 5 fractions? 👀
Ferrario et al and POPART team reports 100 pts treated with prostate-bed SBRT (32.5 Gy/5 fx):
• Late ≥G2 GU toxicity: 8.5%
• Only 1 late G2 GI event
• Just 3 in-field relapses
• 30-mo biochemical control: 74%
Very encouraging local control and toxicity—but urinary QoL remains important to watch.

Across 3 prospective studies, the signal for post-prostatectomy SBRT in 5 fx is increasingly consistent: feasible, low toxicity & excellent in-field control. StereoBed randomized data show no PRO/toxicity penalty; POPART/esSBRT show few local failures.
