By that logic, the kidney never cared about rib resection, large incisions, or flank bulges. Yet we embraced laparoscopy and its steeper learning curve to reduce morbidity. Progress requires the courage to evolve, not the comfort of staying put.

Honest choice. Port count is a means, not the goal. The right platform is whichever lets you control the hilum and close the defect without heroics. Nephron-sparing is a fair test of that: the kidney doesn't care how many holes it took.

Honored that Dr. Mani Menon highlighted my video of SP precision #prostatectomy during his Andrew Novick Innovative Award reception lecture at @CleClinicUro SP symposium a giant in robotic surgery whose relentless pursuit of innovation and excellence continues to inspire us all!

Robotic radical nephrectomy. You've dissected the hilum, and the renal artery is rigid, circumferentially calcified, no give when you palpate it with the instrument.

Lots of good ideas, in my case clips DID work (eventually) but plaque crushing was critical. I also consulted my vascular surgery friends and had them in the room while doing this.
Amazing video! Did many of these cases with @RoboticsUrology during fellowship. The SP robot allows us to have a very different view of the management of these hilar / central tumors

Amazing how SP can expand options for our patients. A 3-cm retroperitoneal incision in the supine position allowed complete removal of a complex staghorn stone in a single setting—with only 45 minutes of operative time. This is what MIS innovation is about. @HUMCUrology

A privilege to have Dr. Menon join us @HUMCUrology and provide valuable guidance during “prostate de-escalation surgery” using the SP robot. This may preserve continence and erections without compromising cancer control in select patients, unlike focal therapy. @MMenonMD

Anterior approach with minimal apical/urethral mobilization can achieve continence comparable to Retzius-sparing, with potentially favorable oncologic margins—without highly selected patients. And it can be performed effectively extraperitoneally with the SP robotic system.

Single port partial nephrectomy update: ROSI has been hard to source lately, so we've moved to the Boehringer Suction Device (BSSD) for some cases. Video below shows it in use.
Congratulations @JayChavaliMD incredible accomplishment
I’m humbled to share that I have now completed our first 100 Single-Port (SP) robotic urology cases at MUSC.
What started as an effort to build a focused program in surgical innovation has grown into a truly unique SP robotic urology practice at MUSC and in South Carolina. The growth has been phenomenal, and it has been incredibly rewarding to see patients from across all corners of our state gain access to advanced minimally invasive surgical care.
This milestone is not simply about the number of cases. It reflects the trust of our patients, the dedication of our operating room teams and colleagues, and a shared commitment to innovation while always keeping patient safety and outcomes at the center of every decision.
I am deeply grateful to everyone who has helped build this program and supported this journey. One hundred cases is an exciting first milestone—but there is still much to learn, improve, and accomplish.
Looking forward to continuing to grow thoughtfully and responsibly as we work toward the next landmark: 1,000 SP robotic urology cases.

The best treatment for POP is robotic sacrocolpopexy, with mesh advancement addressing ant and post defects. With the SP robot (3cm incision), with proper training, often takes under an hour. Unfortunately, many URPS surgeons remain inadequately trained in the robotic approach.

We’re seeing this more and more with referral for SP robotic simple.Recently, a patient came from a nationally renowned center after PAE and radiation for prostate cancer, yet 12–15 stones >1 cm were overlooked, leaving him miserable. Now we’ll do SP robotic simple+stone removal.
What’s going on in some centers? 🤔
A patient with a history of Rezūm and TURP is still voiding terribly, despite alfuzosin + finasteride. And now, as a third step, he’s being offered prostatic artery embolization (PAE). 😲
What about #HoLEP? Why not?
We should offer patients what they need—not just what we have or what we know how to do. 😀

Appendix is not 100% reliable. Sometimes skimpy, sometimes obstructed, but when good, it’s a great, well vascularized solution for R sided radiated ureters. Here I’m using a 8cm appendix onlay flap to fix a long, radiated (XRT+brachy for CaP) mid-distal ureter. See ICG/NIRF vids! – at Philadelphia, PA
