Every company developing G12X drugs should pay attention to this data at #ESMO26 Zecdarasib (G12Di) plus chemo in 1L metastatic PDAC, phase 1 ORR 89% with mFOLFIRINOX (n=27)
ORR 69% with gem/nab-paclitaxel (n=26)
Awesome safety for early DE. For context, other early 1L PDAC response rates (cross-trial which we would neeeeever do):
Zoldonrasib + FOLFIRINOX: 82%
Zoldonrasib + gem/nab-P: 61%
Daraxonrasib + gem/nab-P: 59%
Daraxonrasib alone: 47% Chemo clearly still matters. Starting to think about this like Breakwater… In PDAC, monoallelic inhibitors are extremely effective, just more effective with a little extra oomph behind them. They will land in adjuvant, neoadjuvant, metastatic and every space you can imagine in the next few years. Question is do we combo single plus pan or single plus chemo, and what is the added agent (especially in CRC)? @OncoAlert @TheGutOncLab @Onco_Nexus @Incyte

G12Di INCB161734 (zecdarasib; @Incyte) showed🔥ORRs of 89% w mFFX (n=27) and 69% (n=26) w GnP in 1L mPDAC, w/o concerning safety signals, presented today at #ESMO26. 👏 Adds to other👍early ORR data in 1L mPDAC - zoldon/FFX: 82% - zoldon/GnP: 61% - darax/GnP: 59% - darax: 47%

Replying to @ilyassahinMD
We have two papers. First is small subgroup analysis of IPREDICT— now in press. Second is larger near 100,000 patient analysis. Not accepted yet. Both show TMB 16 threshold. Threshold is independent of tumor type.

History never repeats but it rhymes!
GenFleet's October 2026 investor presentation reports clinical activity with the chemotherapy-free combination of GFH375 (KRAS G12D inhibitor) and cetuximab (EGFR antibody) in metastatic colorectal and pancreatic cancers, following earlier acceptance for full ESMO 2026 late-breaking abstract submission. Heavily pretreated mCRC (n=54): - ORR: 44% - DCR: 89% - Median PFS: 7.3 months (median follow-up 5.4 months) - 94% treated in third line or later - 50% treated in fourth line or later - 65% remained on treatment at data cutoff First-line KRAS G12D-mutant PDAC (n=24): - ORR: 71% - DCR: 100% - 88% remained on treatment - 96% had metastatic disease - Median follow-up: 3.8 months, with only three PFS events What cetuximab may add, monotherapy vs combination: Metastatic CRC (both previously treated): - ORR: 11% → 44% - DCR: 71% → 89% - Monotherapy n=28; combination n=54 PDAC (different treatment settings): - ORR: 40.7% → 71% - DCR: 96.7% → 100% - Monotherapy n=59 (previously treated); combination n=24 (first-line) The contribution of cetuximab is particularly interesting in CRC, where the combination's higher response rate may reflect suppression of adaptive EGFR signaling. In PDAC, differences in treatment line limit direct comparison. These are nonrandomized cohorts. Tolerability (n=78): - Grade 3 treatment-related adverse events: 24% - Treatment interruptions: 21% - Dose reductions: 5% - Treatment-related discontinuations: 0% - No Grade 4–5 treatment-related adverse events GI events were frequent, predominantly low-grade, with limited information on their evolution over successive treatment cycles. US experience with VS-7375 🇺🇸(the same molecule as GFH375 🇨🇳): GI adverse events are predominantly front-loaded, attenuating after the first cycle. Administration with food and prophylactic antiemetics have improved tolerability. An improved formulation is under development, with no vomiting observed in preliminary canine testing, aiming to further improve tolerability without compromising antitumor activity. Why chemotherapy-free treatment matters: real-world attrition and patient choice. Metastatic CRC, Danish nationwide registry (n=7,202): - 46.4% never initiated first-line systemic treatment - 47.6% of treated patients reached second line - 20.3% of treated patients reached third line Metastatic PDAC, 2026 US real-world analysis (n=2,708): - 35% received no systemic therapy - 52% of patients starting chemotherapy discontinued within 90 days - 34% of chemotherapy starters discontinued within 90 days without documented disease progression Metastatic PDAC, Netherlands nationwide study (n=13,756): - 72% received no systemic therapy - Patient or family preference accounted for 39% of reported reasons for withholding systemic treatment Daraxonrasib Phase III, RASolute 302: - 15.1% randomized to chemotherapy never initiated treatment - 2.8% randomized to daraxonrasib never initiated treatment The burden of chemotherapy is real. Patients can tolerate only so much treatment, and its physical and personal costs can limit what remains possible. The potential of chemotherapy-free regimens extends beyond avoiding cytotoxic therapy: durable disease control while preserving patient fitness, treatment continuity and future therapeutic options. @FAndreMD @EileenMOReilly @Aiims1742 @MPishvaian #KRAS #G12D #GFH375 #PDAC #ESMO26 📎 genfleet.com/Public/Uploads/…

Cancer vaccines are being explored not only to help prevent cancer from returning, but also to prevent it from developing. At #ESMO26, Dr. Eduardo Vilar-Sanchez will present research on a vaccine designed for people with Lynch syndrome, an inherited condition that raises the risk of several cancers. Tango reports 82% tumor response rate in trial of combo therapy (Daraxonrasib plus vopimetostat) for advanced #PancreaticCancer at #ESMO26 statnews.com/2026/10/23/esmo… Reduced toxicity data is particularly important as a lower Daraxonrasib dose can be used.

Part 3: 98% pathological response | 95% MPR | 68% pCR | 100% 3-year DFS. Part 4: ctDNA clearance, organ preservation and unanswered questions. Take-home: Short-course neoadjuvant immunotherapy shows exceptional promise, but surgery omission remains unproven. “Chemo brain” is one of the most common things my patients ask me about. We still have no proven drug to treat it. In our small phase II trial, breast cancer survivors with cognitive complaints took oxaloacetate for 8 weeks. Every participant who completed the study reported improvement in cognition, along with less fatigue and better physical function. It’s an early signal, not an answer. A randomized, placebo-controlled trial is the next step. Led by Dr. Patti Ganz, with thanks to @BCRFcure. Out now in @JCOOA_ASCO. doi.org/10.1200/OA-26-00063 #bcsm

🚨: The @FDA has granted accelerated approval to the immunotherapy Jemperli (dostarlimab) for adults with stage 2 or 3 rectal cancer whose tumors have specific genetic characteristics known as mismatch repair-deficient (MMRd) or microsatellite instability-high (MSI-H). The approach, developed at MSK by gastrointestinal medical oncologists Dr. Andrea Cercek (@AndreaCercek) and Dr. Luis Diaz Jr. (@ldiaz1971), uses immunotherapy alone to harness the body’s own immune system to fight cancer. This allows some patients to skip standard treatment for rectal cancer, which includes surgery, radiation, and chemotherapy, and can have life-altering consequences for bowel and urinary function, as well as fertility.
