This critical review examines tools for predicting #CART toxicities, highlighting EASIX for severe CRS and ICANS and CAR-HEMATOTOX for prolonged cytopenia. Integrating clinical scores, CAR-T kinetics and imaging biomarkers may improve risk stratification. astctjournal.org/article/S26…

EPCORE DLBCL-2 Wow 😮 A lot of benefit—and a lot of $$$. HR 0.49 is enormous: a 51% reduction in progression or death with epcoritamab + R-CHOP in frontline higher-risk DLBCL. This is not a marginal improvement and could redefine treatment. But epcoritamab alone may add approximately $250,000 per patient at current US list price. Now we need the curves: absolute PFS benefit, NNT, durability, OS, toxicity—and which biological subgroups benefit most. The signal is unquestionably impressive. The full dataset will tell us whether treating everyone is justified. #DLBCL #HemeTwitter

Ok, looks like Epco-R-CHOP is ready for primetime. HR 0.49 is impressive, and will probably be the new SoC. What we eagerly wait: 1. Toxicity data 2. How Glofi-Pola-R-CHP will perform in the ph3 trial 3. Molecular data Exciting times ahead!

With a plethora of options now for early relapsed (1-3 prior LoT) #Myeloma who are either naive or exposed/sensitive to anti-CD38-mAb (except CART-4 which had 25% CD38-refractory), how does the MRD-negative CR rates compare across regimens? While BsAbs/CAR T remains at the top, impressive performance of Iber-Dara-Dex considering ~50% Len-refractory and 90% IMiD-exposed population. Remains to be seen how this translates into PFS! #IMS26

In a phase 1 study of anito-cel, a CAR T-cell therapy, for relapsed or refractory myeloma, all 38 patients had a response. Severe cytokine release syndrome and immune effector cell–associated neurotoxicity syndrome were rare. The Other Renal Amyloids: It’s not ALL about AL Amyloid by @vsanchorawala Amyloidosis isn’t synonymous with AL. Renal differential ➡️ ATTR, AA, ALECT2, AFib, AApoAI, AApoAIV, AApoCII, ALys. Typing the precursor protein determines treatment, prognosis and genetic implications.

Pancreatic cancer is notoriously hard to treat, but we are making progress. Recent @theNCI -funded work that sheds light on mechanisms behind #daraxonrasib resistance and could help clinicians choose treatment combos that outsmart pancreatic tumors. go.nih.gov/AcvCu95

Another chapter of #downwithdex, this time for AL amyloidosis! By the book, dex 20-40 mg QW for 6 months in ANDROMEDA. No need for this in real life. No ∆ in efficacy; rise in CHF exacerbations and ⬆️ diuretics with prolonged dex. I do 4-8 weeks of 12-20 mg then stop.
#Myeloma Paper of the Day: Limited duration (≤3 months) of dexamethasone in newly diagnosed AL #amyloidosis (w/ daratumumab-based regimen) results in higher 6-month CR rates (66.7 vs 30.4%, p<0.001) and reduced toxicity, similar organ responses: pubmed.ncbi.nlm.nih.gov/4256…. #alsm
