101 experts from 25 countries | New international consensus published in Urologic Oncology.
I am pleased to share our recent publication in Urologic Oncology Seminars and Original Investigations :
“Project international Delphi consensus recommendations for perioperative management of inguinal lymphadenectomy in cancer patients”.
Developed within the Penile Cancer Optimization Initiative (PeCOI), @GTumors and @CAU_URO , this study addresses the prevention and management of perioperative complications following inguinal lymphadenectomy, an area where considerable variability in clinical practice remains.
Key findings:
* 101 experts from 25 countries, representing a broad international and multidisciplinary panel.
* 86 clinical statements evaluated through a two-round modified Delphi process.
* Consensus achieved on 51 statements (59.3%), covering preoperative optimization, lymphatic complications, drain management, antibiotic and thromboembolic prophylaxis, wound complications, and functional rehabilitation.
* Identification of areas of clinical uncertainty requiring further research and prospective validation.
This study provides practical, consensus-based recommendations to support the standardization of perioperative care in inguinal lymphadenectomy, particularly relevant in penile cancer, where high-quality evidence remains limited.
It has been a privilege to work alongside such an outstanding group of internationally recognized experts, @LuisMedinaMD, @drmarcostobias , @jteoh_hk , Amandeep Arora , @drgaganprakash , @DoctorSotelo , and @SpiessPhilippe , as well as all the specialists who contributed to this international consensus.
My sincere thanks to everyone involved, with special thanks to Philippe Spiess, Marcos Tobias-Machado, @ARRODRIGUEZMD , and Luis G. Medina for their support and trust throughout this project.

In a trial involving adults with Ph-positive acute lymphoblastic leukemia, 24-month event-free survival was 86% with the TKI–immunotherapy combination ponatinib–blinatumomab and 67% with imatinib plus chemotherapy. Full phase 3 GIMEMA ALL2820 trial results: nej.md/4scorw0
Editorial: Chemotherapy-free Treatment of Ph-Positive ALL nej.md/3VIVUC8

ALT A graph from an Original Article published in the New England Journal of Medicine article titled "Ponatinib–Blinatumomab or Imatinib–Chemotherapy for Adult Ph-Positive ALL.” The graph shows event-free survival (the primary end point) in the intention-to-treat analysis among patients in the intervention group (who received ponatinib plus blinatumomab) and in the control group (who received imatinib plus chemotherapy). The trial name, GIMEMA ALL2820, is in parentheses after the title. The NEJM identity sits at the bottom.
Such an important topic!

Right on @isliquidbiopsy 🙌🏻
Time for change….
We don’t often think about the utility of liquid biopsy (cfDNA) in myeloid malignancies but it certainly has a role, particularly for capturing the molecular heterogeneity of extramedullary disease.
Read our recent @BCD_AACR paper here for more on this topic.

🧬 New in Blood Cancer Discovery: multiomic profiling of myeloid sarcoma reveals distinct extramedullary clonal evolution, transcriptional adaptation, and recurrent RAS pathway activation.
Importantly, ctDNA sequencing captured the molecular composition of myeloid sarcoma, supporting a potential noninvasive approach to profiling extramedullary AML.

Childhood cancer is not merely a disease we treat—it is a disease we can cure with early diagnosis, timely treatment, and equitable access to care.
The goal is not just better survival, but a future where no child is denied the chance to be cured. 💛

Scaling LLMs to reach AGI is "impossible"
A large language model is trained on about 30 trillion tokens, which is roughly 10^14 bytes of text and would take a person about 400,000 years to read.
A 4-year-old child receives about the same amount of data, 10^14 bytes, through vision alone in about 1 year and 10 months.
In his view, intelligence is the ability to learn new tasks quickly or perform them without prior training, as a teenager learns to drive in about 20 hours. Scaling increases stored knowledge, but it does not produce this ability to adapt.

MECOM rearrangement = high-risk disease, regardless of blast count?
A national registry study from Thailand examined 1,248 myeloid neoplasms and identified 15 cases with MECOM rearrangements.
- MECOM rearrangements were rare: 0.9% of AML and 3% of MDS
- Outcomes were poor, with a 1-year survival of only 27.5%
- Among AML cases treated with intensive 7+3 chemotherapy, none achieved complete remission
- In contrast, all 3 MDS patients treated with hypomethylating agents achieved a complete haematological response
- One patient who subsequently underwent allogeneic stem cell transplantation achieved durable remission
The bigger picture:
The similarly poor survival across lower- and higher-blast disease supports recognizing MECOM-rearranged myeloid neoplasms as a biologically high-risk entity and is not one defined simply by blast percentage.
