🫀 GLP-1: From Glucose Control to Cardiovascular Protection GLP-1 receptor agonists (GLP-1RAs) have officially moved beyond diabetes therapy — they are now cardiovascular drugs. This comprehensive 2026 review in J Clin Invest synthesizes preclinical and clinical evidence showing that GLP-1 signaling exerts direct vascular and cardiac protection, far beyond glucose lowering. 🔬 In experimental models, GLP-1RAs: Reduce atherosclerotic plaque size Stabilize plaques (↑ fibrous cap thickness, ↓ inflammation) Suppress foam cell formation Reduce systemic inflammatory signaling Improve post-MI cardiac function Protect against ischemia/reperfusion injury Improve HFpEF phenotypes (↓ fibrosis, ↓ hypertrophy, ↑ diastolic function) Importantly, many of these benefits appear independent of glucose, cholesterol, or weight loss. 📊 In clinical trials (CVOTs): ~14–15% relative reduction in 3-point MACE Significant reductions in CV death, MI, stroke 14% reduction in heart failure hospitalization SELECT trial: 20% MACE reduction in obese patients without diabetes The emerging message is clear: GLP-1RAs are not just metabolic drugs — they are anti-atherogenic, anti-inflammatory, and cardioprotective agents. 💡 Mechanistically, benefits likely stem from: Plaque stabilization Systemic inflammation reduction Improved myocardial metabolism Favorable body fat redistribution Modest BP and lipid effects ⚠️ Uncertainties remain in HFrEF, but data in HFpEF and ASCVD are compelling. We are witnessing a paradigm shift: Treating cardiometabolic disease by targeting shared inflammatory and metabolic pathways. GLP-1 is no longer just about HbA1c. It’s about vascular biology.

1/4 ANEMIA IS NOT A LIST OF DIAGNOSES We’re taught to approach anemia by pattern. We use mechanisms implicitly. We rarely articulate them. Few are taught to layer probability onto physiology.

Can you get the diagnosis when it comes to carotid stenosis? Having some confusion about the strokes you see w/ICA occlusion? Why are there so many different patterns of infarcts w/ICA occlusion/stenosis? There are 5 main infarct patterns—and they reflect the different types of disruption to flow you get w/carotid disease: artery to artery emboli, embolic shower, & hypoperfusion Here’s what you see: 1.  Territorial infarction w/cortical or subcortical involvement --From artery-to-artery or distal embolism. --More than one occlusion indicates more proximal arterial dz   2.  Subcortical infarction --When ICA, MCA, & ACA perforating deep arteries are occluded --Can be from MCA occlusion w/good collaterals or emboli directly to deep perforating arteries   3. Combined territorial & smaller cortical and/or subcortical infarcts --From partial embolism fragmentation or emboli shower of various sizes   4. Multiple small infarcts in the distal territories of the MCA & ACA --If in more than one arterial territory, it’s suggestive of cardio-embolism --If single arterial territory, it’s from a parent artery steno-occlusive lesion   5. Watershed infarctions. Two types: --External border-zone (located between the cortical territories of ACA, MCA, & PCA) --Internal borderzone (between the deep & superficial networks of MCA & ACA) --Traditionally thought from low-flow, but emboli are often a cause --Hemodynamic mechanism may be more important in internal borderzone infarcts & embolism in external borderzone infarcts
