To put in quotes exactly what the parent or patient says is the chief complaint and work from there. Doctors should actually listen to their patients and the parents of their patients.

The patient is NOT "a poor historian." The patient isn't a historian at all! The patient is the primary source.
YOU are the historian.
Hey Interns, here are The Four D’s of your Assessment and Plan:

New interns: my only advice is to do the physical exam. Do it for real. Push on all of those bellies. Figure out how hard you have to push for people to say "ow" just because you were pushing, and when it's pathologic. Get in the habit of doing a real exam and never stop.

@GE_Appliances Monogram built-in 48in fridge has required multiple services in two short years, extended warranty now one month expired and you won’t cover the service charge on a $10k item that has yet another issue? Do the right thing - take responsibility for your product.

#EmergencyMedicine is a tough job
Once--maybe twice--a year, a moment comes along that sustains you in this career.
I had one of those moments last night.
I am so very grateful to the knowledge sharing of #medtwitter that can make a lifesaving difference.

90% of the physical exam is in talking to the patient. It is a neuro exam, an ENT and pulmonary exam, most of the perfusion (cardiovascular) exam, a skin and muskuloskeletal exam, all at the same time!

So . . . Hear me out . . . It's to watch the disease process evolve. You see a patient, you assess them, you come up with a plan, you implement the plan, you stick around to see if it works or not. You do that a thousand times and you become wise.
Rate, rhythm, axis… really?!?!?

And here’s a little secret, your doctor looks up answers to things ALL THE FUCKING TIME. You think I know how to treat syphilitic chorioretinitis off the top of my head? Trust me, you want a doctor whose first instinct when they don’t know an answer is to look it up
