News from #ASTRO26: Short-course radiation therapy better preserves physical function before pancreatic cancer surgery
Randomized phase 2 trial finds better patient experience with five-treatment option; longer chemoradiation did not significantly improve tumor response or reduce progression.
As seen in @CancerNetwrk, @ASCOPost, @AuntMinnie, & @Medscape. Presented by William A. Hall, MD, from the Medical College of Wisconsin.

An important topic that is frequently discussed during many GI tumor boards.
How do we conceptualize/treat oligoprogressive and OMD?
Thank you @KrishanJethwa for mentoring and working on this with me!

I went to a few of the poster sessions and things were definitely buzzing! Lots of engagement! Fantastic location with easy access to beverage service. 👍
It was really great to see the mix of participants too, from med students to senior faculty to industry. My @MCWRadOnc colleague @RohanPatelMD was so in demand at his station that I could barely say hi to him.
There’s always things to improve upon, but I thought it was a success 👏
Mentors like @DrSpratticus make a world of a difference for your career, and so grateful to have him always in my corner, and to be able to learn and grow as junior faculty with @whallradonc at MCW! And having #ASTRO26 to get a chance to see your people again makes it the best❤️

Awesome catching up with @alison_tree @angela_jia_ @HimanshuNagarMD and @RadOncUH former resident @RohanPatelMD who is faculty at MCW with @whallradonc
All of them continue to inspire me!

This is unprecedented speed and precision.
@lauren_henke crushing it @ASTRO_org and showing the world something they have never seen before.
87% speed improvements and can treat a full online adaptive pancreas and 2 high dose liver SBRTs in under 50 minutes full end to end instead of over 3 hrs on ethos today with all the breath holds.

It’s easy to criticize RT trials for aspirational effect sizes (I’ve done it myself). But these studies v difficult to accrue ($, tech avail off trial).
A smaller effect size can make N infeasible.
Alternative endpoints/stats = potential solutions, but this is current reality.

GI003 HCC proton vs photon: no dif OS, PFS, Gr 3+ AEs.
Protons = 1 option in expanding RT toolbox (MRgRT, adaptive, etc) to optimize therapeutic ratio, & each case should be individualized by tumor & liver fxn.
But w neg RCT & $/access dif, photon RT should remain SOC. #ASTRO26

ASTRO's Annual Meeting allows you to explore the ways new technology, such as AI, help shape the future of radiation oncology.
Ahead of #ASTRO26, we’re asking our members why they are attending ASTRO’s 68th Annual Meeting.
Last up: Leah Katz, MD, MPH, talks about what she is most looking forward to at ASTRO 2026 and what excites her about the future of AI in #radonc.

Short-course RT + IO in rectal cancer. Several recent RCTs w high CR rates (esp notable given lower BED of 25Gy/5fx).
Hypothesis: larger fx enhance antitumor immunity & shorter tx duration reduces RT lymphotox.
IMO data compelling enough for large RCT w organ intent. @OncoAlert

2) Getting to dunk your chair (@whallradonc) & vice chair (Dr. Johnstone) as a fresh junior faculty? ✅✅✅✅

Here is a quick 3 min update on NRG GI-011, highlighting Amendment 2.0 changes:
1. Liberalize post-induction chemo CA199 criteria
2. Allow tumor treating fields
3. Up to 8 mo induction chemo
We hope these changes further facilitate accrual.
Thanks to all for your support 🙏!

A new publication reports a 30-day mortality of 11% among nearly 1,000 pts who underwent histotripsy for primary and metastatic liver tumors. This statistic signals a need for greater scrutiny to clarify the utility and practice patterns of this supposedly "less invasive" treatment.

PRO analysis of PLATO anal ca trials. Major early (6-mo) findings:
1) Dose-escalated RT (ACT5): no clear PRO detriment
2) Reduced-dose RT (ACT4): signal for better sexual fxn
3) Baseline stg appeared to drive post-tx symptom burden
