Cancer care is complex and life changing. Yet in this case the “consultant” is not medically qualified and not on the GMC register, despite using the title Dr on the hospital website. Full transparency about professional background should be standard. See below 👇🏼

Are there some senior RCP folks who might be able to advise which types of staff fall into which Tier? Explicit reference to medical registrar level at Tier 2, in Feb 2026. I fear we have not learned very much. – at Bickleigh, England

A senior surgeon found to have sexually and racially harassed trainees over years, even during live operations, is back at work after a 12 month 'time out' Courts say striking him off was 'disproportionate'. Disproportionate to who? independent.co.uk/news/healt…

Cancer services are being told they can’t hire doctors while patients face long waits and doctors are left unemployed. Doctors who want to diagnose and treat cancer are being stopped at multiple stages from helping patients. The Government caps how many doctors are allowed to train as specialists like radiologists and oncologists (specialists who read and diagnose from scans and treat cancer). Thousands apply each year, and many are rejected because there simply aren’t enough training jobs. Last year, 4,011 doctors applied for clinical radiology training. Only 356 were accepted. For the few who do manage to specialise, it doesn’t get easier. In 2025, half of the UK’s specialist cancer centres were banned from recruiting oncologists, and over a third of radiology departments were blocked from hiring radiologists due to recruitment freezes, despite rising demand and high waiting lists. So we now have an under-doctored country where: - Doctors are turned away from specialist training - Qualified cancer doctors are prevented from working due to recruitment freezes - Patients wait longer for diagnosis and treatment This is not an accident. It’s the result of government caps and cost cutting as well as poor workforce planning. And the jobs crisis is one of the reasons why resident doctors have recently voted to extend their strike mandate. This is not a workforce mystery. It is the predictable result of government caps and NHS cost cutting.

🏴‍☠️ This is the value of nursing as a profession these days: Just qualified as a nurse and unsure if to do ACP or Graduate entry medicine. ⭕️ A Couple of ACPs I spoke to said they do all things even the consultants do including even surgery in ENT but they just don't get the same wages. ⭕️ ACP is around 60k whereas eventually reaching consultant level is 140k. ⭕️ It's not about the money but you wouldn't have to work full time so would be less stressed overall on higher pay. Absolutely shambolic.

It’s explicitly asking them to make diagnoses (despite not being medically qualified), and asking them to replace a consultant histopathologist in MDTs where we deliberate on cancer diagnosis I think using “consultant” in medical settings while being non-medical is misleading 1/

There have been some conversations around how ACPs are supposedly safer than PAs, and are better trained, and so can work at ST3 level even if PAs are <F1 level as per Leng. I thought it’d be useful to compare the ACP courses to the PA courses 1/

I would love to continue the fight on doctor substitution for situations like this. Sadly, I have been abruptly removed from the BMA working group on this with very little notice.

There would be no need to make any luck at all if training was treated as a critical activity in that scenario, but it's not, and thus the weird workarounds develop, with more 'successful' people being the ones who are able to push their luck further and 'escape better'.

Basically by refusing to protect training from other stuff, we're actually selecting people who are better at avoiding or escaping from 'other stuff', or that care less about the consequences of doing so - up to a point where that balancing act tips or fails.

It's a bit disturbing that if you as a consultant are failing to train your residents that the answer *must* be that they're not plucky enough, or not making their own luck. Intervene, speak to the management and get them off the ward rota, then train them.

It's the same argument all the way down resulting from taskification and separating cognition from task (as per Dr Vaughan). The consultant surgeons in this case need to be insisting that the residents are off the ward and in theatre *much* more, and train them!

This seems like an extremely apathetic self-defeating logic circle, because we also *insist* that the ward work is a crucial part of learning to be a surgeon, but don't for the SCP, who is effectively being trained to do surgery without the same demands.
