I really admire figures that can describe a concept better than long manuscripts. This image is from an excellent paper in Nature by Vivek Subbiah about the future of evidence-based medicine and the design of clinical trials, all inspired by the rise of artificial intelligence. This is the era of the next generation of 'deep' medicine. "Current paradigms must be continuously challenged by emerging technology and by all stakeholders (the new generations of scientists, physicians, the pharma industry, regulatory authorities and, most importantly, patients). Disruptive innovation should lead to every clinical site being a research site, with all necessary quality checks and research as part of the standard of care. My personal view is that every major disease needs a ‘Moonshot’ program and every rare disease should have an ‘Operation Warp Speed’—both with clearly identified, sustainable goals to improve population health and address equity, diversity and global access to therapies. Methodological advances and future AI-based analyses of all data will provide deep evidence to realize the goal of personalized medicine— that is, to offer the right treatment to the right patient at the right time." BREAKING: BULLSEYE! Trump's nemesis Rosie O'Donnell enrages MAGA world by predicting that his movement will totally collapse once Trump is either "arrested" or no "longer with us." And we can't argue with her logic... "Some of the things he's been doing lately... You know, every time you think he can't go lower than this, we've reached the bottom of the barrel, this is it, he digs another crevice and jumps right in," she told Jim Acosta on his podcast. "You know I don't understand why he still has any followers." "But I do feel that when he is either arrested or no longer with us, so will the MAGA movement, dissolve in a similar way," she said. "I think there's no one there with the kind of free press and charisma that he had." We couldn't agree more. Right now, Trump's two likeliest heirs are Marco Rubio and JD Vance. The former is a charmless warmonger with nothing to offer the American people beyond more costly bloodshed and mindless fealty to Israel. The latter is a whiny, repulsive coward who can't make it through a rally these days without someone interrupting to condemn his complicity in the Epstein coverup. Trump, for all his evil, incompetence, stupidity, and cruelty, knows how to work a crowd. He possesses a showman quality that allowed him to build this movement on lies and bluster. Rubio and Vance are empty suits who lack the instincts to tap into the fascist id that's driving this movement. The delicious irony here is that Trump himself is obsessed with legacy. It's why he's always trying to slap his name on buildings and erect monuments to himself. On some level, he's aware that he'll leave no achievements behind and his movement will crumble to ash. "On 'The Apprentice' which was a fictional show sold as fact and people believed that and those people I think will be the last ones to wake and they will eventually," added O'Donnell. "And it's our job as citizens of the United States to help everyone heal together when this is over. And hopefully in 13 days, it's the beginning of the end of this horrible reign of terror." Once again, she nailed it. The midterms are going to be the end of the Trump era and when Democrats retake Congress, it will neuter him as President. Please ❤️ and share if you can't wait to see the end of MAGA!

Coronary Calcium Is Changing the Rules of Lipid Management: Treat the Plaque, Not Just the Risk Score! Article: Imaging Guided Treatment of Subclinical Atherosclerosis—We've Come a Long Way! Authors: Eugenia Gianos, MD, and Ron Blankstein, MD. Journal: Journal of Clinical Lipidology, 2026;20:11–14. 12 Clinically Important Pearls 1\. Risk scores alone are no longer the whole story. The 2026 ACC/AHA dyslipidemia guideline integrates PREVENT-ASCVD risk estimation, risk enhancers and selective coronary artery calcium (CAC) imaging to personalize lipid-lowering therapy. 2\. CAC scoring receives a Class I recommendation. In men aged ≥40 years and women aged ≥45 years with 10-year PREVENT-ASCVD risk of 3% to less than 10%, CAC scoring is recommended when the decision regarding lipid-lowering therapy remains uncertain. 3\. CAC = 0 does not mean zero risk. Statin deferral may be considered in selected individuals, but should not be automatic in patients with diabetes, familial hypercholesterolemia, active smoking or a strong family history of premature ASCVD. 4\. CAC 1–99: Consider moderate-intensity statin therapy, particularly when CAC is ≥75th percentile. The suggested LDL-C goal is below 100 mg/dL. 5\. CAC 100–299: Initiate LDL-C-lowering therapy, targeting LDL-C below 70 mg/dL. 6\. CAC 300–999: Target LDL-C below 70 mg/dL. A more intensive LDL-C goal below 55 mg/dL may be considered, particularly in selected high-risk patients. 7\. CAC ≥1000: Extensive coronary atherosclerosis warrants intensive lipid lowering, targeting LDL-C below 55 mg/dL, with an optional ApoB goal below 55 mg/dL. 8\. CAC may underestimate plaque burden. Patients with diabetes, HIV, inflammatory disorders or markedly elevated Lp(a) may have substantial noncalcified coronary plaque. Selective coronary CT angiography carries a Class IIb recommendation. 9\. Never ignore incidental CAC on chest CT. Coronary calcification detected during CT examinations for noncardiac indications provides valuable cardiovascular risk information. Moderate-to-severe incidental CAC supports considering moderate- to high-intensity statin therapy with an LDL-C goal below 70 mg/dL. 10\. The VESALIUS-CV trial strengthens primary prevention in selected high-risk patients. Evolocumab demonstrated cardiovascular benefit in patients with high-risk diabetes or atherosclerosis without previous myocardial infarction or stroke. 11\. Lower for longer is becoming increasingly important. Earlier and sustained reduction in LDL-C may reduce cumulative exposure to atherogenic lipoproteins and potentially prevent the development or progression of subclinical atherosclerosis. 12\. CAC is a decision-making tool, not a universal screening test. Patients who already have a clear indication for lipid-lowering treatment should not have therapy delayed merely to obtain a CAC score. CME INDIA – Take-Home Message The absence of cardiovascular symptoms does not imply the absence of coronary atherosclerosis. The emerging approach is to combine clinical risk estimation with selective plaque imaging, identify subclinical disease earlier and match LDL-C targets to atherosclerotic burden. Remember: CAC = 0 is not a lifetime guarantee of cardiovascular protection, while CAC ≥300 identifies substantial subclinical coronary disease that deserves serious consideration of intensive lipid lowering. doi.org/10.1016/j.jacl.2026.… CME INDIA – Learning Today, Improving Patient Care Tomorrow.

NEW imagining study (REACT STUDY) on SILENT ATHEROSCLEROSIS in over 16,000 adults without a history of established CVD. *Prospective ongoing study but this data is from a single time point (cross-sectional)* All subjects underwent "three-dimensional vascular ultrasonography of the carotid and femoral arteries and with coronary computed tomographic angiography". Some early call outs: 1 - There was a BIG change in atherosclerosis prevalence from age 30 to 40. Almost triple the number of male subjects at age 40 had measurable atherosclerosis in at least one of the 3 measured territories compared with subjects at age 30. This looks like a very important decade. At age 40 average coronary plaque volume was 50mm3 (it's unclear in the methods section how this was quantified - need to dig into it more). Also interesting at age 40, male and female calcium score is similar - around 10. But from this point they diverge with male calcium score being significantly greater than female calcium score each decade thereafter. 2- A clean CT Angiogram doesn't = no atherosclerosis. In fact middle age men were more likely to have carotid plaque. 3 - Overall women tend to have much less absolute plaque volume, however this isn't normalised for vessel size. I'd like to see a comparison of stenosis. A great new data set to explore. Cc @NutritionMadeS3 @Drlipid @ethanjweiss @RonKarlsbergMD @RonBlankstein @BudoffMd
