Impact of COVID-19 on gastrointestinal disease in vaccinated and unvaccinated individuals: a population-based cohort study in England 🚨COVID leaves a years-long GI scar, even after mild cases, far worse after hospitalisation. Vaccination cuts the damage sharply. New OpenSAFELY data on 18M+ people👇 ➡️This interesting OpenSAFELY population-based cohort study (linked NHS data on ~18–24 million English adults) examined incidence of 10 gastrointestinal diseases (upper/lower GI bleeding, pancreatitis, appendicitis, GERD, dyspepsia, peptic ulcer, NASH, gallstones, IBS) after COVID-19 diagnosis versus before/without it. ➡️The study analysed three separate cohorts to compare gastrointestinal disease risk after COVID-19 according to vaccination era and status: 1. Pre-vaccination cohort (January 2020 – June 2021; n ≈ 18.4 million): - This period captured the highest elevations, - After hospitalised COVID-19, the adjusted hazard ratio (aHR) for upper gastrointestinal bleeding was 20.1 in weeks 1–4 and remained elevated at 1.90 in weeks 53–102, - After non-hospitalised COVID-19 the corresponding aHRs were 1.92 and 1.34, - Similar patterns were seen across the other nine gastrointestinal outcomes, - Excess risk of upper GI bleeding at 28 weeks was approximately 159 per 100,000 people diagnosed with COVID-19, - Elevated incidence persisted for up to two years, driven mainly by severe/hospitalised cases. - Period when the Wuhan/Alpha variant was dominant. 2. Vaccinated cohort (June – December 2021; n ≈ 14.9 million): - aHRs were consistently lower than in the other two cohorts, - For upper GI bleeding in weeks 1–4 the overall aHR was 1.84; after hospitalised COVID-19 it was 15.2 and after non-hospitalised disease it was 1.20, - Excess risk of upper GI bleeding at 28 weeks fell to about 60 per 100,000, - Vaccination attenuated both the early peak and the longer-term elevation, - Period when the Delta variant was dominant (until Omicron became dominant near the end of follow-up). 3. Unvaccinated cohort (June – December 2021; n ≈ 3.5 million) - Risks were higher than in the vaccinated group and closer to the pre-vaccination era, - For upper GI bleeding in weeks 1–4 the overall aHR was 4.26, after hospitalised COVID-19 it was 19.7 and after non-hospitalised disease it was 2.14, - Excess risk of upper GI bleeding at 28 weeks was approximately 112 per 100,000, intermediate between the pre-vaccination and vaccinated cohorts, - Period when the Delta variant was dominant (until Omicron became dominant near the end of follow-up). ➡️Across all three cohorts the elevation was greatest after hospitalised COVID-19, increased with age, and was attenuated by prior vaccination. ➡️The longest persistence (up to two years) was observed in the pre-vaccination cohort. ‼️So, severe COVID-19 leaves a years-long elevated risk of gastrointestinal disease that vaccination meaningfully blunts but does not erase. ‼️Again, the vaccinated were the smartest! Data above support AGAIN vaccination as an important public health intervention in reducing severe COVID-19 outcomes and its possible consequences! ‼️At population scale this translates into substantial extra GI bleeding, reflux, pancreatitis and related diagnoses for years after infection, concentrated among older people and those hospitalised. It increases outpatient, endoscopic and hospital burden, especially in high-prevalence periods, and underscores why preventing severe COVID-19 remains clinically relevant long after the acute phase. #Mitigation #AvoidSars2 #AvoidReinfections #UpdateVaccination bmjpublichealth.bmj.com/cont…

Spain maintained substantial baseline SARS-CoV-2 sequencing and rapidly scaled up in response to PJ.2.1. This novel saltation variant is spreading internationally, raising questions about the protection provided by updated vaccines and potential differences in host-cell interactions compared with previous variants. A strong example of why sustained genomic surveillance matters. #PJ21 🇪🇸

Het eerlijke verhaal waarom Zweden voor Novavax heeft gekozen. Zweden heeft via Adda Inköpscentral een officiële aanbesteding uitgeschreven voor Covid-19 vaccins voor seizoen 2027/2028.
Sweden opts for coronavirus protein vaccine instead of mRNA From the 2027/2028 season onwards, Sweden will use only a protein-based coronavirus vaccine for its national vaccination campaign. It is the first country in the world to say goodbye to the mRNA vaccines. Difference in technology Nuvaxovid is a classic protein vaccine. It contains a ready-made piece of the virus (the spike protein), produced using recombinant technology (a molecular method in which DNA from different organisms is combined in the laboratory and placed into a host cell (such as a bacterium or yeast) to allow it to multiply or produce proteins) and provided with an adjuvant (an excipient that stimulates and strengthens the body's immune response to the antigen, so that the vaccine works better). The body is presented with this protein directly to build an immune response. This differs from mRNA vaccines (such as Comirnaty from Pfizer/BioNTech), where the body's cells must produce the spike protein themselves after receiving the mRNA instruction. This process carries the risk of autoimmune reactions, especially because these products do not remain at the injection site but can spread throughout the body. Furthermore, the quantity and duration of spike protein production are not defined in the mRNA technique. In addition, the lipid coating of the mRNA vaccines is very reactive. All in all, this leads to a disproportionate number of side effects, some very serious and resulting in death (1-3). Sweden chose its own path during the coronavirus years by not enforcing lockdowns or school closures. Although there was much criticism of this at the time, the Swedish approach was later appreciated ( De Morgen, 25 December 2023 ). With the decision to phase out coronavirus mRNA vaccines, Sweden appears set to play a pioneering role once again. The Doctors' Collective, together with European partners, has been advocating for the cessation of mRNA vaccinations for quite some time.

SARS-CoV-2 Is Evolving to Become Harder for Naïve B Cells to Recognize! ➡️ Researchers found that naïve human B cells were 3 times less likely to recognize the KP.2 RBD than the original Wuhan-Hu-1 RBD. ➡️ Mutations such as #E484A and #F486P appear to reduce recognition by naïve B-cell receptors, potentially narrowing the pool of B cells available to initiate new antibody responses. ➡️ The bigger picture: SARS-CoV-2 evolution may do more than evade existing antibodies—it may also make it harder for the unexposed immune system to recognize its spike protein in the first place.

Keep on dreaming!

"[The hospitalized adult patients in Pennsylvania] are weak and disoriented with meningitis, sepsis or pneumonia. Some have bacterial infections in their blood, kidney failure or hepatitis.. Pennsylvania’s measles outbreak is defying epidemiologists’ expectations and historical trends: Adults account for about two-thirds of the 1,300 cases reported by the state.. Until now, measles was rarely seen among adults.. Pennsylvania’s outbreak is also sending people to the hospital at almost twice the rate seen in other recent outbreaks. Six people diagnosed with measles — four of them adults — have died, according to state health officials and local coroners. An infant, born with the disease after his mother was sick during pregnancy, was among the deaths.. The toll showcases how adults over age 20 are among the groups at greatest risk for serious complications.. Children under 5 and people with compromised immune systems are also at high risk for severe illness.." I believe the number of immunocompromized adults is increasing.
